中国汉族人群抑郁障碍严重程度与儿童期虐待、人格特征和应激性生活事件的相关关系

Relationships between childhood abuse,personality and stress of life events in Chinese Han depressed people

目的探究中国汉族人群抑郁障碍患者儿童期虐待、人格特征、应激性生活事件与抑郁症状的相关性。方法共入组抑郁障碍患者203例(87例男性,116例女性)完成汉密尔顿抑郁量表(HAMD-17),艾森克人格问卷(EPQ),儿童期不良家庭环境调查表(CECA),生活事件量表(LES)的调查。采用SPSS AMOS 22.0统计软件进行相关分析构建结构方程模型。结果结构方程模型拟合指数理想:χ~2(6)=9.98,P=0.13,CFI=0.96,RMSEA=0.06。儿童期虐待对抑郁障碍的总体效应为0.446(标准系数估计值),直接效应0.316,通过人格特征的间接效应为0.130,人格特征作为中介变量调节儿童期虐待与抑郁障碍严重程度的关系。人格特征对负性生活事件的直接效应为0.437,负性生活事件对抑郁障碍无明显影响。结论本研究首次分析了中国汉族人群中儿童期虐待、人格特征、应激性生活事件与抑郁障碍患者抑郁症状的严重程度的关系。儿童期虐待直接或间接影响抑郁障碍的严重程度。

Objective To investigate how childhood abuse,personality,and stress of life events were associated with symptoms of depression in Chinese Han depressed people.Methods Patients with depressive disorder(n=203,87 men and 116 women) completed the Hamilton Depression Scale(HAMD),the Eysenck Personality Questionnaire(EPQ),the Childhood Experience of Care and Abuse(CECA),and the Life Experiences Survey(LES).SPSS AMOS 22.0 was used to construct the structural equation model(SEM).Results SEM revealed χ~2(6)=9.98,P=0.13,CFI=0.96,RMSEA=0.06.The regression weight of childhood abuse on the severity of depression was 0.446,the direct effect was 0.316,indirect effect through personality was 0.130.Childhood abuse indirectly predicted the severity of depression through the mediation of personality.The direct effect of personality on negative life change score of the LES was 0.437.Negative life change score of the LES did not predict the severity of depression.Conclusions This study first report a relationship between childhood abuse,personality,life stresses and the severity of depression in Chinese Han depressed patients.Childhood abuse directly and indirectly predicted the severity of depression.
