颈前入路减压融合内固定术治疗脊髓型颈椎病的临床疗效

CLINICAL EFFECT OF ANTERIOR CERVICAL DECOMPRESSION AND FUSION IN TREATING CERVICAL SPONDYLOSIS MYELOPATHY

目的评估颈前路减压融合手术治疗脊髓型颈椎病(cervical spondylotic myelopathy,CSM)的临床效果。方法回顾性分析2010年6月—2016年9月于该院行颈前路减压融合内固定治疗的CSM患者32例,术前、术后3个月、术后6个月应用量角器测量颈椎矢状平面的活动度(ROM)、视觉模拟评分法(VAS)以及日本骨科协会(Japanese Orthopaedic Association,JOA)评估治疗分数的方法评估术后恢复情况,计算末次随访即术后6个月神经功能改善率。结果随访6个月,所有患者均未出现严重并发症,ROM及VAS评估均术前明显改善,术前JOA评分(9.44±2.61)分,术后3个月(13.06±2.11)分,术后6个月(15.34±1.38)分,与术前比较,差异有统计学意义(P<0.05),改善率(81.01%±13.35)%。结论颈前路减压融合内固定术治疗脊髓型颈椎病(CSM)效果显著,值得推广。

Objective To evaluate the clinical outcome of anterior cervical decompression and fusion in treating cervical spondylosis myelopathy.Methods Thirty-two cases suffer from cervical spondylosis myelopathy were treated with anterior cervical decompression and fusion in author's hospital from Jun 2010 to September 2016.The clinical outcomes were evaluated by the parameters including symptoms,pain score,improvement of Japanese Orthopaedic Association(JOA)score postoperation,and improvement rate 3months after operation and at the last follow-up.Results Thirty-two cases were followed up for 6months,no serious complications occurred.JOA scores were increased from(9.44±2.61)pre-operatively to(13.06±2.11)after three months and(15.34±1.38)at the final follow-up(P<0.05).After treatment,postoperative ROM and VAS scores were obviously improved than that of preoperative.Conclusion Anterior cervical decompression and fusion in treatment of cervical spondylotic myelopathy is effective,it is worth in clinical widely and reference.
