慢性心力衰竭证型与心率变异性的关系

Correlation Between Chinese Medical Syndromes and Heart Rate Variability in Chronic Heart Failure

为探讨慢性心力衰竭 (CHF)中医辨证分型与心率变异性 (HRV)的关系 ,提供慢性心力衰竭临床辨证参考指标。选择 76例慢性心力衰竭患者按中医辨证分为心气阳虚、气虚血瘀、心肾阳虚、阴虚水犯、心阳虚脱五型 ,2 4例正常健康人作为对照组 ,进行相关指标检测。结果 :CHF患者HRV(SDNN ,SDANN )较正常对照组降低 (P <0 0 5或0 0 1) ;各中医证型组中从心气阳虚、气虚血瘀、心肾阳虚、阳虚水犯 ,到心阳虚脱各组HRV呈依次降低趋势 ,组间差异显著 (P <0 0 5或 0 0 1)。提示 :CHF患者HRV部分指标变化可作为判定CHF严重程度的参考指标 。

To probe the correlation between Chinese medical syndromes and heart rate variability (HRV) in chronic heart failure (CHF) and offer some objective indexes for Chinese medical differentiation, 76 CHF cases were differentiated into five syndromes: heart-yang deficiency syndrome, qi-deficiency and blood-stasis syndrome, heart-kidney yang-deficiency syndrome, yang-deficiency and water-overflow syndrome, and heart-yang collapse syndrome, and another 24 healthy people were observed as control. HRV-related indexes were detected. Results: CHF cases had lower HRV (SDNN, SDANN) than healthy people (P<0.01, P<0.05); the HRV (SDNN, SDANN) of the five syndromes decreased in sequence in above order, with differences between any two syndromes (P<0.01, P<0.05). It is revealed that HRV can signify the severity of CHF and can be used for the references of Chinese medical differentiation.
