动脉颈内静脉血气监测评价重症患者脑代谢功能 被引量:5

摘要 目的 探讨临床评价危重患者脑组织代谢功能的简便、实用、快速、有效的观察方法。方法  2 8例昏迷患者为观察组 ,2 5例清醒患者为对照组 ,同步进行动脉、颈内静脉双份血样标本的血气分析。结果 经对比分析 ,观察组的动脉与颈内静脉血氧分压差、氧含量差均低于对照组 (P <0 0 1) ;氧饱和度差也低于对照组 (P <0 0 5 ) ,脑氧摄取率降低 ,将其作为预后不良的指标。动、静脉氧分压差的灵敏度为 71 4 3% ,特异度为 72 % ;血氧饱和度差与血氧含量差的灵敏度均为 5 7 14 % ,特异度均为 88%。结论 动脉、颈内静脉血气分析血氧差异的监测对判断昏迷患者脑组织摄氧、代谢状态、评价疗效、判断预后均有临床实用价值 ,优于单份标本血气分析。
