腔内激光联合手术治疗下肢静脉功能不全208例报告

Study on DIOMED laser treatment with or without concomitant surgical operation of superficial varicosities in the lower limbs

目的:评价腔内激光联合手术治疗下肢静脉功能不全的临床研究。方法:208例病人,男93例,女115例,共230条患肢均行激光或激光联合手术治疗。主要有4种手术治疗方式:①单纯激光治疗15例,19条肢体;②激光治疗联合高位大隐静脉结扎术76例,94条肢体;③激光治疗联合股浅静脉瓣膜包窄术和大隐静脉高位结扎术共112例,112条肢体;④激光治疗联合大隐静脉高位结扎和交通支结扎术5例,5条肢体。结果:随访2～27个月,平均6.12个月,208例病人均疗效满意恢复正常的生活自理或工作,无局部复发。结论:静脉腔内激光治疗是一种新的治疗方法,具有安全、微创、疗效好、操作简便和不遗留手术疤痕等优势.联合手术治疗时,可扩大治疗应用范围。

Objective To evaluate the results of endovenous treatment of superficial varicosities in lower extremities with DIOMED laser. Methods Two hundred and thirty lower limbs in 208 patients (93 males and 115 females) underwent laser therapy with or without concomitant surgical intervention. Four therapeutic entities were included: 1 simple endovenous laser therapy: 19 limbs in 15 patients; 2 endovenous laser therapy combined with high ligation of great saphenous vein: 94 limbs in 76 patients; 3 endovenous laser therapy combined with external banding valvuloplasty of superficial femoral vein and high ligation of great saphenous vein: 112 limbs in 112 patients; 4 endovenous laser therapy combined with high ligation of great saphenous vein and perforator(s): 5 limbs in 5 patients. Results All patients were followed up 2-27 months (mean 6.12 months) with satisfactory results, no recurrence was observed. Conclusion Endovenous laser therapy is a new therapeutic entity possessing advantages of safety, effectiveness, minimal invasiveness, easy technology and without leaving any scar. It can be used in combination with surgery as well.
