摘要
目的:调查北京地区健康体检人群中男性骨密度的情况,为男性骨质疏松症的防治提供参考。方法:选择2017年1月~2018年12月在中日友好医院健康体检中心进行健康体检的男性,排除继发性骨质疏松症及其他影响骨代谢的因素后共2067例纳入研究。年龄20~79岁,按年龄段分为6组。测量受试者腰椎1~4正位及股骨颈的骨密度进行描述,同时对同一年龄组不同部位的骨量异常(包括骨量减少及骨质疏松)患病率进行对比。结果:北京地区健康体检男性人群腰椎骨密度峰值在20~29岁,股骨颈骨密度峰值在30~39岁。随着年龄增长,股骨颈骨密度逐渐下降,腰椎骨密度在60岁之前逐步下降,60岁以后反而出现上升的情况。50~59岁男性虽然腰椎骨密度的绝对值仍在下降,但腰椎骨量异常的患病率低于50岁以下男性。60岁以下男性腰椎骨量异常患病率明显高于股骨颈,70岁以上男性股骨颈骨量异常的患病率明显高于腰椎。结论:对于50岁以上中老年男性,在测定腰椎骨密度的同时应参考股骨颈骨密度。50岁以下中青年男性,应早期预防骨质疏松症。
Objective:To investigate the bone mineral density(BMD)of healthy males in Beijing area and to provide a reference for the prevention and treatment of osteoporosis in men.Methods:A total of 2067 males who underwent physical examination in the Health Hxamination Center of China-Japan Friendship Hospital from January 2017 to December 2018 were selected,and secondary osteoporosis and other factors affecting bone metabolism were excluded.The subjects were aged from 20 to 79 years old,and were divided into 6 groups according to the age of every 10 years.The BMD of the subjects in the 1~4 of lumbar spines and femoral neck were measured and described,and the prevalence of bone mass abnormalities(including osteopenia and osteoporosis)in different parts of the same age group was compared.Results:The peak BMD of lumbar was in men between 20~29 years old,and the peak BMD of femoral neck was between 30~39 years old.With the increase of age,the BMD of femoral neck decreased gradually;the BMD of lumbar decreased gradually before the age of 60,but increased after the age of 60.The prevalence of abnormal bone mass in the lumbar in males under 60 years old was significantly higher than that in the femoral neck,and the prevalence of abnormal bone mass in the femoral neck in males over 70 years old was significantly higher than that in the lumbar spine.Conclusion:Lumbar spine BMD should be measured together with femoral neck BMD in aged men over 60 years old.For early prevention of osteoporosis,the BMD in young and middle-aged men under the age of 60 years should be paied enough attention to.
