柳氮磺胺嘧啶对大鼠心脏移植缺血再灌注损伤心肌保护作用研究

摘要 目的 分析大鼠心脏移植后缺血再灌注损伤中IL 1 β、MPO、ICAM 1表达及组织损伤情况 ,探讨柳氮磺胺嘧啶 (SSA)对移植心脏的保护作用。方法 建立大鼠异位心脏移植模型 :正常SD大鼠心肌为对照组 ,SD到Wistar大鼠为同种移植组 ,同种移植SSA治疗组。心肌组织标本进行免疫组化和病理学检查 ,检测ICAM 1表达和心肌损伤 ,分别用放射免疫法和多媒体彩色图文分析系统对IL 1 β、MPO和ICAM 1进行定量测定。结果 对照组IL 1 β、MPO、ICAM 1表达均微弱 ;而同种移植组IL 1 β、MPO和ICAM 1明显增高 ,SSA治疗组中IL 1 β、MPO、ICAM 1表达较同种移植组明显降低 ;对照组心肌组织结构正常 ,同种移植组中性粒细胞浸润 ,出现心肌间质水肿和组织坏死 ;SSA治疗组心肌损伤程度明显降低。结论 大鼠移植心脏缺血再灌注损伤中IL 1 β、MPO、ICAM 1含量明显增高 ,心肌组织损伤 ,经SSA治疗后含量显著降低。 Objective To investigate the protective effect of SSA on transplanted rat heart,by establishing improved rat heart transplant model and using SSA to treat myocardial ischemic reperfusion injury,quantitatively measuring the expression of IL 1β,MPO and ICAM 1 in transplanted myocardium tissue and the injury of the transplanted heart.Methods A rat heterotopic cardiac transplant model with SD donors and Wistar recipients was used.Control group of normal SD rat heart; Group of SD to Wistar rat heart transplantation; Group of SD to Wistar heart transplantation and treated with SSA.Harvested grafts received immunohistochemical and histopathological examinations to evaluate the expression of ICAM 1 and the injury of myocardium tissue.Radioimmunoassay and multimedia pathology imaging analysis system were used for quantitative measurment of IL 1β,MPO and ICAM 1 expression.Results Faint IL 1β,MPO and ICAM 1 expression in control group;in heterotopic transplant group,IL 1β,MPO and ICAM 1 content significantly elevated; after treatment of SSA,IL 1β,MPO and ICAM 1 content significantly decreased.In control group the cardiac tissue were normal;There were many lymphocytes infiltrated in,myocardium stromatic edema and tissue necrosis can be found in heterotopic transplant group; the injury of grafts was relived significant by SSA.Conclusion IL 1β,MPO and adhesion molecule ICAM 1 excessively express in grafts,SSA can suppress the acute graft rejection and protect the transplanted rat hearts.
