糖化血红蛋白对2型糖尿病合并高血压患者颈动脉粥样硬化的影响分析 被引量:2

摘要 目的探究2型糖尿病合并高血压患者糖化血红蛋白(HbA1c)水平对于颈动脉粥样硬化的影响。方法选取36例2型糖尿病合并高血压患者作为观察组,另选取40例健康志愿者作为对照组。对两组的各项临床生化指标、血压及颈动脉进行检测并作比较;同时将观察组患者根据其糖化血红蛋白水平不同分为A组(糖化血红蛋白水平≥6.5%)和B组(糖化血红蛋白水平≤6.5%),比较两组颈动脉粥样硬化斑块发生情况。结果观察组的收缩压、舒张压、空腹血糖、血清超敏C反应蛋白水平均显著高于对照组,差异具有统计学意义(P<0.05)。观察组的颈动脉粥样硬化斑块发生率为75.0%,显著高于对照组的40.0%,差异具有统计学意义(P<0.05)。A组的颈动脉粥样硬化斑块发生率为86.96%,明显高于B组的53.85%,差异具有统计学意义(P<0.05)。结论 2型糖尿病合并高血压患者颈动脉粥样硬化的发生率更高,同时糖化血红蛋白水平越高的患者其颈动脉粥样硬化的发生率也越高。 Objective To discuss the effect of glycosylated hemoglobin(HbA1 c) on carotid atherosclerosis in patients with type 2 diabetes mellitus and hypertension. Methods There were 36 patients with type 2 diabetes and hypertension as observation group, and another 40 healthy volunteers were selected as control group. The clinical biochemical indexes, blood pressure and carotid artery were measured and compared between the two groups. At the same time, the patients in the observation group were divided into group A(glycosylated hemoglobin level ≥6.5%) and group B(glycosylated hemoglobin level <6.5%) according to different glycosylated hemoglobin levels, and the carotid atherosclerotic plaques were compared between the two groups. Results The observation group had significantly higher systolic blood pressure, diastolic blood pressure, fasting blood glucose, serum high-sensitivity C-reactive protein levels than the control group, and their difference was statistically significant(P<0.05). The observation group had significantly higher incidence of atherosclerotic plaque as 75.0% than 40.0% in the control group, and the difference was statistically significant(P<0.05). Group A had obviously higher incidence of carotid atherosclerotic plaque as 86.96% than 53.85% in group B, and the difference was statistically significant(P<0.05). Conclusion The incidence of carotid atherosclerosis is higher in patients with type 2 diabetes and hypertension, and the higher the level of glycosylated hemoglobin, the higher the incidence of carotid atherosclerosis.
