双柱锁定加压钢板与外固定支架在桡骨远端C型骨折治疗中的应用对比研究

Comparative study of double-column locking compression plate and external fixator in the treatment of type C fracture of distal radius

目的分析双柱锁定加压钢板与外固定支架在桡骨远端C型骨折治疗中的应用效果。方法 55例桡骨远端C型骨折手术治疗的患者,按入院时间顺序分为探究组(28例)和参照组(27例)。探究组采用双柱锁定加压钢板治疗,参照组采用外固定支架治疗。比较两组患者治疗后3个月腕关节活动度及腕关节X线测量、功能评分情况。结果两组掌倾角、尺偏角比较差异无统计学意义(P>0.05);探究组腕关节功能评分(4.84±2.58)分显著低于参照组的(7.53±2.84)分,差异有统计学意义(P<0.05)。探究组旋前角为(74.35±13.47)°、旋后角为(73.84±12.91)°、背伸角为(40.32±13.01)°、掌曲角为(42.06±13.64)°;参照组旋前角为(56.74±11.20)°、旋后角为(55.94±11.07)°、背伸角为(26.43±5.71)°、掌曲角为(23.75±7.43)°;探究组旋前角、旋后角、背伸角、掌曲角均大于参照组,差异有统计学意义(P<0.05)。结论将双柱锁定加压钢板应用于桡骨远端C型骨折治疗中效果显著,且短时间内其治疗效果优于外固定支架治疗,临床治疗时应依据患者实际情况选择手术治疗方式。

Objective To analyze the effect of double-column locking compression plate and external fixator in the treatment of type C fracture of distal radius. Methods A total of 55 patients with type C fracture of distal radius were divided by admission order into investigation group(28 cases) and control group(27 cases). The investigation group was treated with double-column locking compression plate, and the control group was treated with external fixator. The wrist joint mobility and wrist X-ray measurement and functional scores in the two groups after 3 months of treatment was compared. Results Both groups had no statistically significant difference in palm inclination and ruler deviation(P>0.05). The investigation group had significantly lower wrist joint function score as(4.84±2.58) points than(7.53±2.84) points in the control group, and the difference was statistically significant(P<0.05). The investigation group had pronation angle as(74.35±13.47)°, supination angle as(73.84±12.91)°, back extension angle as(40.32±13.01)°, palm angle as(42.06±13.64)°, which were(56.74±11.20)°,(55.94± 11.07)°,(26.43±5.71)° and(23.75±7.43)° in the control group. The investigation group had larger pronation angle, supination angle, back extension angle and palm angle than the control group, and their difference was statistically significant(P<0.05). Conclusion Double-column locking compression plate shows remarkable effect in treating type C fracture of distal radius, and its treatment effect is better than that of external fixator in a short time. The surgical treatment should be selected according to the actual situation of patients in clinical treatment.
