超声引导下置管引流术在卵巢恶性肿瘤术后淋巴管囊肿治疗中的应用价值

Application value of ultrasound-guided catheter drainage in the treatment of lymphatic cyst after ovarian malignant tumor operation

目的通过对卵巢恶性肿瘤外科术后淋巴管囊肿行超声引导下置管引流术,探讨其在卵巢恶性肿瘤外科术后淋巴管囊肿治疗中的临床价值。方法 27例卵巢恶性肿瘤外科术后淋巴管囊肿患者,行超声引导下置管引流术。术后3 d、7 d及2周对其复查超声,观察治疗效果及患者术后并发症发生情况。结果 27例患者淋巴管囊肿均一次性超声引导下置管成功,引出液体量105.0～487.0 ml/d,平均引出液体量(185.6±113.7)ml/d。患者术后随访2周,淋巴管囊肿消失,下肢肿胀疼痛症状消失,治愈出院。27例患者中,因1例多房性淋巴管囊肿合并感染,在置管引流后、囊内注入抗生素治疗3 d, 7 d复查囊腔缩小, 2周后复查囊腔完全闭合。除1例合并感染者超声引导下置管过程中有疼痛感外,余患者均未见疼痛等发生,所有患者经超声引导下置管引流治疗后无并发症发生。结论超声引导下置管引流术对卵巢恶性肿瘤外科术后淋巴管囊肿是一种有效的治疗手段,可以为临床治疗卵巢恶性肿瘤术后淋巴管囊肿提供一种新的治疗方式。

Objective To discuss the clinical value of ultrasound-guided catheter drainage in the treatment of lymphatic cyst after ovarian malignant tumor operation. Methods Ultrasound-guided catheter drainage was performed in 27 patients with lymphatic cyst after surgery for malignant ovarian tumors. Ultrasound was reviewed at 3 d, 7 d and 2 weeks after operation to observe the treatment effect and the occurrence of postoperative complications. Results 27 patients with lymphatic cysts were successfully placed under the guidance of one-time ultrasound. The volume of liquid drained was 105.0~487.0 ml/d, and the average volume of liquid drained was(185.6±113.7) ml/d. After 2 weeks of follow-up, lymphatic cysts disappeared, swelling and pain of lower limbs disappeared, and the patients were cured and discharged. Among the 27 patients, 1 case of multilocular lymphatic cyst complicated with infection. After catheter drainage, the capsule was injected with antibiotics for 3 d. The cysts were retracted at 7 d after treatment, and the cysts were completely closed after 2 weeks of treatment. No pain occurred in patients except for 1 patient with infection who had pain during ultrasound-guided catheterization. No complications occurred after ultrasound-guided catheterization and drainage. Conclusion Ultrasound-guided catheter drainage is an effective treatment for lymphatic cysts after surgery for ovarian malignant tumors. It can provide a new treatment for lymphatic cysts after surgery for ovarian malignant tumors.
