摘要
目的探讨改变给药次序对提高子宫输卵管超声造影诊断符合率的研究价值。方法 400例因不孕到本院妇产科就诊并行子宫输卵管超声造影的患者为研究对象,随机分为A组和B组,每组200例。在不改变药物及剂量的前提下, A组患者先注入混合液后打造影剂, B组患者先打造影剂后注入混合液。比较两组患者的诊断符合率。结果 A组患者中由于异位妊娠已手术切除一侧输卵管10条,腹腔镜下美兰通液术诊断390条输卵管,其中输卵管通畅160条,通而不畅178条,阻塞52条。子宫输卵管超声造影检查符合343条,诊断符合率为87.9%。B组患者中由于异位妊娠已手术切除一侧输卵管15条,腹腔镜下美兰通液术诊断385条输卵管,其中输卵管通畅153条,通而不畅150条,阻塞82条。子宫输卵管超声造影检查符合357条,诊断符合率为92.7%。B组患者的诊断符合率高于A组,差异具有统计学意义(P<0.05)。结论先注入造影剂后注入混合液的造影给药方式可提高子宫输卵管超声造影的诊断符合率。
Objective To discuss the research value of changing the order of administration in improving diagnostic coincidence rate of hysterosalpingography. Methods A total of 400 cases of infertility patients in gynecology and obstetrics department for hysterosalpingography as study subjects, were randomly divided into group A and group B, with 200 cases in each group. Without changing the drug and dosage, group A was injected with mixed solution before shadowing agent, group B was injected with mixed solution after shadowing agent. The diagnostic coincidence rate in two groups was compared. Results In group A, 10 fallopian tubes had been removed because of ectopic pregnancy, and 390 fallopian tubes were diagnosed by laparoscopic Melan hydrotubation, of which 160 tubes were unobstructed, 178 tubes were partially obstructed and 52 tubes were obstructed. There were 343 accorded tubes by hysterosalpingography, and the diagnostic coincidence rate was 87.9%. In group B, 15 fallopian tubes were resected and 385 fallopian tubes were diagnosed by laparoscopic Melan hydrotubation. Among them, 153 tubes were unobstructed, 150 tubes were partially obstructed and82 tubes were obstructed. There were 357 accorded tubes by hysterosalpingography, and the diagnostic coincidence rate was 92.7%. Group B had higher diagnostic coincidence rate than group A, and the difference was statistically significant(P<0.05). Conclusion The diagnostic accuracy of hysterosalpingography can be improved by injecting mixed solution after shadowing agent.
