半躺式哺乳姿势在剖宫产术后母乳喂养中的应用

目的探讨半躺式哺乳姿势在剖宫产术后母乳喂养中的应用效果。方法 200例实施剖宫产的产妇,利用随机组别的形式分为对照组与观察组,每组100例。对照组剖宫产术后在护士的帮助下进行传统坐位横抱式母乳喂养姿势,观察组剖宫产术后由护士协助产妇进行半躺式哺乳姿势并进行母乳喂养的技术指导。比较两组产妇术后第1、2、3天的伤口疼痛评分、哺乳体位舒适度评分、母乳喂养率、乳头皲裂率、乳汁淤积率。结果观察组术后第1、2、3天的伤口疼痛评分及哺乳体位舒适度评分均优于对照组,差异具有统计学意义(P<0.05)。观察组产妇乳头皲裂率与乳汁淤积率分别为6.0%、9.0%,均低于对照组的19.0%、22.0%,差异具有统计学意义(P<0.05)。观察组产妇产后第1、2、3天母乳喂养率分别为97.0%、93.0%、89.0%,均高于对照组的86.0%、81.0%、76.0%,差异具有统计学意义(P<0.05)。结论在剖宫产术后产妇中应用半躺式哺乳姿势,可以明显减轻伤口疼痛,降低乳头皲裂和乳汁淤积发生率,提高产妇哺乳舒适度和母乳喂养率,故值得在临床上进一步推广和应用。

Objective To discuss the application effect of semi-lying breast-feeding posture in breastfeeding after cesarean section.Methods A total of 200 maternal women with cesarean section were divided by random grouping method into control group and observation group,with 100 cases in each group.In the control group,traditional breast-feeding posture was performed with the help of nurses after cesarean section,while in the observation group,the nurses assisted the parturients in semi-lying breast-feeding posture after cesarean section and provided technical guidance for breast-feeding.Comparison were made on wound pain score,lactation position comfort score,breastfeeding rate at 1,2 and 3 d after operation nipple splitting rate and milk deposition rate between the two groups.Results The observation group had better wound pain score,lactation position comfort score at 1,2 and 3 d after operation than the control group,and the difference was statistically significant(P<0.05).The observation group had lower nipple splitting rate and milk deposition rate respectively as 6.0%and 9.0%than 19.0%and 22.0%in the control group,and their difference was statistically significant(P<0.05).The observation group had higher breastfeeding rate at 1,2 and 3 d after operation respectively as 97.0%,93.0%and 89.0%than 86.0%,81.0%and 76.0%in the control group,and their difference was statistically significant(P<0.05).Conclusion Application of semi-lying breast-feeding posture in parturients after cesarean section can obviously relieve wound pain,reduce nipple splitting rate and milk deposition rate,and improve the lactation position comfort and breast-feeding rate.It is worthy of further promotion and application in clinic.
