ICU中血清降钙素原定量检测与血培养结果的对比分析 被引量:1

摘要 目的对比分析重症监护室(ICU)中血清降钙素原(PCT)定量检测与血培养结果。方法 500例ICU疑似感染患者,分别行血清PCT定量检测与血培养。以血培养结果作为金标准,对比两种方法血流感染诊出率;分析PCT定量检测的敏感度、特异度、准确率。结果 500例患者中,细菌感染47例,血培养诊出47例,诊出率为100.00%;PCT定量检测诊出44例,诊出率为93.62%,比较差异无统计学意义(P>0.05)。以血培养作为金标准, PCT定量检测的敏感度为93.62%(44/47)、特异度为100.00%(453/453)、准确率为99.40%(497/500)。结论血清PCT定量检测在ICU细菌感染中的应用效果确切,对细菌感染的敏感性较高,能够早期提示细菌感染严重程度,为临床早期抗生素治疗提供可靠依据。 Objective To compare and analyze the results of quantitative detection of serum procalcitonin(PCT)and blood culture in intensive care unit(ICU).Methods Quantitative detection of serum PCT and blood culture were performed in 500 patients with suspected ICU infection.With the results of blood culture as gold standard,the diagnostic rate of bloodstream infection was compared between the two methods,and the sensitivity,specificity and accuracy of PCT quantitative detection were analyzed.Results There were 47 cases of bacterial infection in 500 patients,and 47 cases were diagnosed by blood culture,with diagnostic rate as 100.00%;44 cases were diagnosed by quantitative detection of PCT,with diagnostic rate as 93.62%.The difference was not statistically significant(P>0.05).With blood culture as gold standard,the sensitivity of quantitative detection of PCT was 93.62%(44/47),specificity was 100.00%(453/453),and the accuracy was 99.40%(497/500).Conclusion Quantitative detection of serum PCT shows definite effect in bacterial infection in ICU,and has a high sensitivity to bacterial infection.It can early indicate the severity of bacterial infection and provide a reliable basis for early clinical antibiotic treatment.
