临床药师主导的慢性病管理在提高老年糖尿病患者对药物认知中的作用分析

Analysis of the role of clinical pharmacist-led chronic disease management in improving the drug awareness of elderly diabetes mellitus patients

目的探究临床药师主导的慢性病管理在提高老年糖尿病患者药物认知程度与治疗依从性中的作用。方法 80例老年糖尿病患者,依据管理方式不同分为对照组和观察组,每组40例。对照组患者采用常规慢性病管理方式,观察组患者采用临床药师主导的慢性病管理方式。比较两组患者管理前后糖尿病药物认知程度与服药依从性。结果管理前,两组患者糖尿病药物认知程度评分比较差异无统计学意义(P>0.05);管理后,两组患者糖尿病药物认知程度评分均高于本组管理前,且观察组糖尿病认知程度评分(90.52±1.64)分高于对照组的(75.37±1.48)分,差异具有统计学意义(P<0.05)。观察组患者服药总依从率为97.50%,显著高于对照组的75.00%,差异具有统计学意义(P<0.05)。结论在老年糖尿病患者中开展由临床药师主导的慢性病管理,可提高患者对治疗药物的认知程度,进而提升服药依从性,值得临床推广应用。

Objective To investigate the role of clinical pharmacist-led chronic disease management in improving the drug awareness and treatment compliance of elderly diabetes mellitus patients.Methods A total of 80 elderly diabetes mellitus patients were divided by different management methods into control group and observation group,with 40 cases in each group.The control group received conventional chronic disease management,and the observation group received clinical pharmacist-led chronic disease management.The cognitive degree of diabetes medicines and medication compliance before and after management was compared between the two groups.Results Before management,there was no statistically significant difference in cognitive score of diabetes medicines between the two groups(P>0.05).After management,both groups had higher cognitive score of diabetes medicines than those before management,and the observation group had higher cognitive score of diabetes medicines as(90.52±1.64)points than(75.37±1.48)points in the control group.Their difference was statistically significant(P<0.05).The observation group had significantly higher total medication compliance rate as 97.50%than 75.00%in the control group,and the difference was statistically significant(P<0.05).Conclusion Clinical pharmacist-led chronic disease management in elderly diabetes mellitus patients can improve patients’awareness of therapeutic drugs,thus improving medication compliance.It is worthy of clinical promotion and application.
