人绒毛膜促性腺激素与妊娠早期甲状腺功能亢进症相关性分析 被引量:12

The correlation analysis on human chorionic gonadotropin hyperthyroidism(hCG)in early pregnancy patients

摘要 目的探讨人绒毛膜促性腺激素与妊娠早期甲状腺功能亢进症相关性。方法分析2013年1月-2014年6月在我院接受治疗的41例早期妊娠期甲状腺功能亢进症患者的临床资料,并列为观察组,选取43例在我院进行孕期检查的孕妇作为对照组。结果观察组患者血清中TSH(t=3.919,P<0.001)、FT3(t=14.26,P<0.001)水平明显高于对照组,差异具有统计学意义(P<0.05),观察组血清中hCG水平显著高于对照组,差异具有统计学意义(t=62.87,P<0.001)。两组研究对象FT4水平不存在统计学差异(t=1.928,P=0.057);观察组患者血清中TSAb明显高于对照组,差异具有统计学意义(t=10.09,P<0.001)。两组研究对象在TPOAb(t=1.747,P=0.084)、TGAb(t=1.865,P=0.065)水平上差异不具有统计学意义(P>0.05);采用Pearson相关性检验分析观察组患者hCG与TSH、FT3、TSAb相关性发现,观察组患者hCG与FT3(r=0.521,P<0.001)及TSAb(t=0.354,P=0.023)呈现显著正相关,与TSH呈负相关(t=-0.391,P=0.012)。结论 hCG的高分泌水平是引起妊娠早期甲状腺功能亢进的主要因素,具有抑制TSH激素的分泌,促进TSAb合成的作用。 Objective To investigate the correlation analysis on human chorionic gonadotropin hyperthyroidism(hCG)in early pregnancy patients.Methods Analyzing the 41 cases clinical data of early pregnancy patients with hyperthyroidism in our hospital for treatment from January 2013 to June 2014,and listed as observation group,43 cases pregnant women for pregnancy check in our hospital were selected as control group.Results Serum TSH(t=3.919,P<0.001),FT3(t=14.26,P<0.001)levels In the observation group are significantly higher than the control group,the difference is statistically significant(P<0.05),serum levels of hCG in observation group is significantly higher than the control group,difference is statistically significant(t=62.87,P<0.001).FT4 level in Both groups is no statistical difference(t=1.928,P=0.057).serum TSAb in observation group was obviously higher than that of control group,the difference is statistically significant(t=10.09,P<0.001).TPOAb(t=1.747,P=0.084),TGAb(t=1.865,P=0.065)level in two groups of subjects are no statistically significant difference(P>0.05);Using Pearson correlation test to analyze hCG and TSH,FT3,TSAb correlation in observation group,it found that hCG(t=0.354,P=0.023)show significant positive correlation,and negatively correlated with TSH(t=-0.391,P=0.012)with FT3(r=0.521,P<0.001)、TSAb in observation group Conclusion High level secretion of HCG is the main factor that causing thyroid function hyper function in early pregnancy,and it can inhibit TSH hormone secretion,promote the synthesis of TSAb.
