妊娠期糖尿病胰岛素抵抗与铁蛋白及游离脂肪酸的相关性研究

Correlation between insulin resistance and gestational diabetes ferritin and free fatty acids

目的分析探讨高浓度铁蛋白、游离脂肪酸与妊娠糖尿病胰岛素抵抗相关性,以期为妊娠糖尿病的诊疗提供借鉴。方法以自2011年6月到2012年6月入住我院的妊娠糖尿病患者为对象,采用个体匹配,按照1∶1比例选取对照,即调查每一个病例的同时,调查1个年龄相同、孕周相同的孕妇作为对照。对搜集到的资料,用采用卡方检验、条件Logistic回归分析。评价高浓度铁蛋白、游离脂肪酸与妊娠糖尿病胰岛素抵抗相关性与妊娠糖尿病胰岛素抵抗。结果两组患者平均年龄30.50±3.56岁妊娠糖尿病的孕龄、孕次、孕前BMI,比较后无显著差异(P>0.05),胰岛素抵抗指数(HOMA-IR)、HOMA-β细胞分泌功能指数有显著差异(P<0.05)。妊娠糖尿病组的铁蛋白50.30±25.32μg/L,非妊娠糖尿病组的铁蛋白23.60±12.96μg/L,有显著差异(P<0.05),妊娠糖尿病组的游离脂肪酸384.73±29.60μmol/L,非妊娠糖尿病组的游离脂肪酸252.62±19.41μmol/L,有显著差异(P<0.05),以铁蛋白和游离脂肪酸数值为因变量,以患者的一般资料为自变量,进行条件Logistic回归分析,发现铁蛋白、游离脂肪酸,OR值分别为1.340,1.030,b为正值,P<0.05。结论妊娠期糖尿病妇女体内存在高浓度铁蛋白、游离脂肪酸,与胰岛素抵抗有关,可能参与妊娠糖尿病的发病。

Objective Analysis and Discussion high concentrations of ferritin,free fatty acids and gestational diabetes insulin resistance,in order to provide reference for diagnosis and treatment of gestational diabetes.Methods since June 2011 to June 2012 in our hospital stay gestational diabetes as an object,using individual matching,1:1ratio to select a control,namely the investigation of each case while investigating the same one age,gestational age pregnant women the same as control.On the data collected by using the chi-square test,Logistic regression analysis.Evaluation of high concentrations of ferritin,free fatty acids and gestational diabetes,insulin resistance and gestational diabetes insulin resistance.Results There were an average age of 30.50±3.56 years old with gestational diabetes gestational age,pregnancies,pre-pregnancy BMI,no significant difference after comparison(P>0.05),insulin resistance index(HOMA-IR),HOMA-βcell secretory function index has significant differences(P<0.05).Gestational diabetes group ferritin 50.30±25.32μg/L,non-GDM group ferritin 23.60±12.96μg/L,there is a significant difference(P<0.05),free fatty acid gestational diabetes group 384.73±29.60μmo l/L,free fatty acids of non-GDM group 252.62±19.41μmo l/L,there is a significant difference(P<0.05),with iron protein and free fatty acid value as the dependent variable,to the patient’s general information for the independent variables,conditional Logistic regression analysis showed that ferritin,free fatty acids,OR values were 1.340,1.030,b is positive,P<0.05.Conclusion The presence of high concentrations of gestational diabetes ferritin,free fatty acids,the incidence associated with insulin resistance,women with gestational diabetes may be involved in vivo.
