CEA、CA724、CA19-9对残胃癌诊断的意义及其相关性分析

目的探讨CEA、CA19-9和CA724联合检测对残胃癌诊断的意义以及其表达水平与肿瘤大小、初次手术的良恶性及3年生存期的相关性。方法取2008年3月至2015年6月期间在吉林大学中日联谊医院住院治疗的残胃癌患者44例作为实验组,选择良性胃病手术复查正常患者44例作为对照组,进行组间和组内对比分析。结果残胃癌血清CEA、CA19-9、CA724表达均高于对照组(P<0.01),3种标志物联合诊断灵敏度52.27%,明显高于对照组(P<0.05).3种标志物表达与残胃癌肿物大小和患者生存期无关(P>0.05).CEA在初次手术为恶性疾病患者中表达高于初次手术为良性患者(P<0.05)。结论 3种肿瘤标志物在残胃癌患者血清中表达增高且联合检验可在不显著降低特异度基础上增加灵敏度,3种标志物与肿物大小、患者生存期无关,CEA在初次手术为恶性疾病患者中表达高于初次手术为良性患者。

Objective the significance of combined detection of CEA,CA19-9 and CA724 in the diagnosis of gastric stump carcinoma and the correlation of the expression level with the size of the tumor,the malignancy of the first operation and the 3-year survival time.Methods 44 patients with gastric stump cancer treated in China-Japan Union Hospital of Jilin University from March 2008 to June 2015 were chosen as the experimental group,and 44 patients with other benign gastric diseases were selected as the control group.Results the expressions of CEA,CA19-9 and CA724 in serum of gastric stump carcinoma were higher than those in control group(P<0.01).The sensitivity of combined diagnosis of the three markers was 52.27%,which was higher than that of the control group.(P<0.05).There was no significant relationship between expression of three markers and tumor size and survival time of gastric stump carcinoma(P>0.05).The expression of CEA in patients with malignant disease is higher than the patients with benigh disease after primary surgery(P<0.05).Conclusion the three tumor markers have the same significance in the diagnosis of gastric stump carcinoma,and the combined test can increase the sensitivity without decreasing the specificity.The three markers have no relation with the size of the tumor and the survival time of the patients.The expression of CEA in patients with malignant disease was higher than that in patients with benign disease after primary surgery.
