摘要
目的研究高频彩色多普勒超声血流显像(CDFI)联合声辐射力脉冲成像(ARFI)对于腱鞘巨细胞瘤(GCTTS)的诊断价值。方法选取2015年1月至2016年12月间本院收治的59例疑似GCTTS患者作为研究对象,全部患者均行CDFI联合ARFI检查,并在本院接受后续手术治疗,以术中所见及术中病理结果为金标准,以ROC曲线参数曲线下面积(AUC)评估CDFI、ARFI及二者联合的诊断价值,计算各项诊断效能指标,分析与金标准确诊结果间的一致性。结果联合诊断结果与金标准确诊结果间具有较高一致性,ARFI具有中等一致性,CDFI具有一致性,P<0.05。CDFI联合ARFI诊断符合率最高达93.22%,灵敏度为94.59%,特异度为90.91%。CDFI与ARFI诊断GCTTS具有中等诊断价值,二者联合诊断GCTTS具有较高诊断价值,P<0.05。结论高频彩超联合ARFI对于腱鞘巨细胞瘤具有较高诊断价值,可为临床诊断、疗效评估等提供较为精准的指导。
Objective To study the diagnostic value of high frequency color Doppler flow imaging(CDFI)combined with acoustic radiation force impulse imaging(ARFI)in giant cell tumor of tendon sheath(GCTTS).Methods from January 2015 to December 2016 in our hospital 59 cases of suspected GCTTS patients as the research object,all patients were treated with CDFI combined with ARFI examination,and follow-up surgery in our hospital,as the gold standard to the pathological results of intraoperative findings and in the area of ROC curve with parameters under the curve(AUC)assessment the value of CDFI,ARFI and the two combined diagnosis,the diagnostic performance index calculation,analysis of the consistency between the results and the gold standard for diagnosis.Results there was a high consistency between the combined diagnostic results and the gold standard diagnostic results.ARFI was of moderate consistency,and CDFI was consistent,P<0.05.The diagnostic accuracy rate of CDFI combined with ARFI was 93.22%,the sensitivity was 94.59%,and the specificity was 90.91%.The diagnosis of GCTTS with CDFI and ARFI is of moderate diagnostic value,and the combined diagnosis of GCTTS with two is of high diagnostic value,P<0.05.Conclusion high frequency color Doppler ultrasound combined with ARFI has high diagnostic value for giant cell tumor of tendon sheath,and it can provide more accurate guidance for clinical diagnosis and evaluation of curative effect.
