类风湿关节炎患者血清14-3-3η蛋白、免疫球蛋白的变化及其意义

Relationship between serum 14-3-3 ETA and immunoglobulin level and the development of RA patients

目的探讨类风湿关节炎(RA)患者血清免疫球蛋白、14-3-3η蛋白的变化及其意义。方法选取我院2015年2月—2017年12月收治的RA患者110例(RA组)和年龄、性别相匹配的健康对象40例(对照组),检测两组的血清IgA、IgG、IgM、补体C3、C4、14-3-3η蛋白水平,并根据RA患者是否处于疾病活动期和放射学分级进行分层分析。结果 RA组的血清IgG、14-3-3η蛋白水平显著高于对照组(P<0.05),血清IgA、IgM、补体C3、C4蛋白水平与对照组比较差异无统计学意义(P>0.05);活动期RA患者血清IgG、14-3-3η蛋白水平显著高于非活动期的RA患者(P<0.05),血清IgA、IgM、补体C3、C4水平与非活动期的RA患者比较差异无统计学意义(P>0.05);放射学分级3级、4级RA患者血清IgG、补体C3、14-3-3η蛋白水平显著高于非活动期组的2级患者(P<0.05),血清IgA、IgM、补体C4水平与放射学分级2级RA患者比较差异无统计学意义(P>0.05)。结论 RA患者血清IgG、补体C3 14-3-3η蛋白水平较高,血清14-3-3η蛋白、免疫球蛋白水平变化与RA患者病情、活动情况具有一定的相关性。

Objective To investigate the changes and significance of serum immunoglobulin and 14-3-3 ETA protein in patients with rheumatoid arthritis(RA).Methods In our hospital(2015.02-2017.12),110 cases of RA patients(group RA),age and sex matched healthy subjects(control group)were selected to detect the levels of serum IgA,IgG,IgM,complement C3,C4,14-3-3 ETA protein in the two groups,and stratified analysis according to whether the patients were in the period of disease activity and the radiological classification.Results Serum IgG and 14-3-3 ETA protein levels in group RA were significantly higher than those in the control group(P<0.05).There was no significant difference in serum IgA,IgM,complement C3 and C4 protein levels between the RA group and the control group(P>0.05),and the serum IgG and protein levels of the RA patients in the active group were significantly higher than those in the non active group.There was no significant difference in serum IgA,IgM,complement C3,and C4 levels between the patients with RA in the dynamic phase group and the RA patients in the inactive group(P>0.05);the serum IgG,complement C3,14-3-3 ETA protein levels of the radiological classification(grade 3,4)were significantly higher than those of the group 2 in the inactive group(P<0.05),and the radiological classification(grade 3)There was no significant difference in serum IgA,IgM and complement C4 levels between patients with RA and those of grade 2 RA(P>0.05)in patients with RA.Conclusion Combined detection of serum IgG and complement C314-3-3 ETA protein level is helpful for clinicians to make judgments on the condition and activity of RA patients,so as to guide treatment.
