白细胞介素-8和分泌性中耳炎的相关性研究

Study on the relationship between Interleukin-8 and secretory otitis media

为探讨白细胞介素 - 8(IL- 8)在分泌性中耳炎发病机制中所起的作用 ,采用酶联免疫吸附试验法对行鼓膜穿刺术的 5 9例分泌性中耳炎患者 71份中耳积液进行 IL - 8浓度检测。结果示中耳积液中 IL - 8水平明显增高 (>7.81pg/ ml) ,检出阳性率为 81.7% ,平均浓度为 2 98.11± 92 .14 pg/ ml。儿童患者 IL- 8平均浓度较成人显著增高 (P<0 .0 1) ,粘液性中耳积液 IL- 8平均浓度较浆液性积液显著增高 (P<0 .0 1)。IL- 8的浓度和病程呈正相关 ,病程长者含量高 (P<0 .0 5 )。本次鼓膜穿刺前进行过药物治疗 (包括抗生素、激素治疗 )和未接受过治疗的患者 IL - 8浓度差异无显著性 (P>0 .0 5 )。提示分泌性中耳炎患者的中耳积液中存在 IL- 8,它对中性粒细胞的趋化作用可以引起中耳粘膜损伤并可导致病变转为慢性。

Using enzyme-linked immunosorbent assay (ELISA), we measured levels of interleukin-8 in middle ear effusion (MEE) from patients with secretory otitis media undergone auripuncture to study the role of cytokines in the pathogenesis of the disease.Significant levels of IL-8 (>7.81pg/ml) were found in 58 cases(81.7%) of 71 MEEs. The mean (±s) levels of IL-8 was 298.11±92.14pg/ml. With respect to MEE types, the mean level of IL-8 in serous effusion was lower than that in mucoid (P<0.01). Meanwhile, the mean level of IL-8 in MEEs from children was higher than that from adults (P<0.01). We've found a positive correlation between the level of IL-8 in MEE and the course of the disease,the ranger the course the higher the level and vice versa. In addition, the amounts of IL-8 in MEEs of patients with medication (antibiotics, hormones) before auripuncture were not statistically significant as compared to those without medication(P>0.05). We have demonstrated the presence of IL-8 in MEEs. The neutrophil chemotactic and activating properties of this cytokine may be responsible for mucosal damages and thus the chronicity of the disease.
