应用OCT检测年龄相关性黄斑变性的分析

Analysis of the value of OCT in detection of age-related macular degeneration

目的光学相干断层成像(optical coherence tomography,OCT)技术在年龄相关性黄斑变性中的应用分析。方法收集在本院健康体检中,自愿应用OCT技术在眼底视网膜黄斑部检查的60岁以上人员1228例,男827例,女401例,视力0.1~1.2。分析OCT技术在检测年龄相关性黄斑变性中的作用。结果 OCT体检1228例,共发现年龄相关性黄斑变性359例,检出率29.2%。其中男827例,检出236例,检出率28.5%;女401例,检出123例,30.7%。年龄60~65岁852例,检出197例,23.1%;66~70岁295例,检出116例,39.3%;>70岁81例,检出46例,56.8%。视力0.1~0.3135例,检出76例,56.3%;视力0.4~0.8 765例,检出218例,28.5%;视力>0.8 328例,检出65例,19.8%。结论年龄相关性黄斑变性是随年龄增长而发病率逐渐上升,同时,视力越低,发病率越高,但与性别关系不大。OCT技术对于年龄相关性黄斑变性的早期病变检出率较高,对患者确定早期治疗方案及预后具有重要指导价值。

Obiectives To explore the application of OCT in detection of age-related macular degeneration( AMD). Methods 1228 subjects aged 60 or older were voluntarily to have an OCT examination for detection of AMD during the health examination were included in this study. There were 827 males and 401 females,with vision acuity of 0. 1 to 1. 2. The data were collected and used to evaluate the significance of OCT in detection of AMD. Results Among the 1228 subjects,359 cases were diagnosed as AMD with a positive detection rate of 29. 2%. Among them,236( 28. 5%) of the 827 males and 123( 30. 7%) of the401 females were diagnosed of AMD. 197( 23. 1%) of the 852 cases with age of 60 to 65,116( 39. 3%) of the 295 cases with age of 66 to 70,46( 56. 8%) of the 81 cases with age greater than 70. 76( 56. 3%) of the 135 cases with vision acuity of 0. 1 to 0. 3,218( 28. 5%) of the 765 cases with vision acuity of 0. 4 to0. 8,and 65( 19. 8) of the 328 cases with vision acuity of 0. 8 were diagnosed as AMD. Conclusions The incidence of AMD is positively correlated with the increase of age and decrease of vision acuity,but not obviously with the gender. OCT technology is able to detect AMD at early stage,as well as the size and severity of the lesions,therefore,it may play a vital role in the timely detection,prevention and treatment of age-related macular degeneration.
