穴位按摩缓解PICC置管患者疼痛的效果观察

Efficacy of acupoint massage for pain relief in patients with peripherally inserted central catheter

目的观察穴位按摩缓解经外周静脉置入中心静脉导管(PICC)患者疼痛的效果。方法将99例PICC置管患者按随机数字表法分为研究组49例和对照组50例。对照组按照常规置管,在扩皮前进行0.1%利多卡因局部麻醉。研究组按照常规置管,在穿刺至撤鞘时间段,助手进行穴位按压(合谷穴、液门穴)。比较2组患者的疼痛积分、血压、心率、一次穿刺成功率和满意度。结果置管后,2组收缩压和心率均升高,但研究组低于对照组(P<0.05);舒张压变化不明显,差异无统计学意义(P>0.05);2组患者各时间点疼痛评分与一次穿刺成功率比较差异无统计学意义(P>0.05);研究组患者满意度(100.00%)显著高于对照组(94.00%),差异有统计学意义(Z=-1.966,P=0.049)。结论穴位按摩能有效稳定PICC置管患者的血压和心率,缓解置管引起的疼痛,提高患者满意度。

Objective To investigate the effect of acupoint massage on pain relief caused by peripherally inserted central catheter(PICC) insertion. Methods Totally 99 patients with PICC were randomly divided into research group(n=49) and control group(n=50). Patients in the research group received local anesthesia with 0.1% lidocaine before expansion of skin, while those in research group were intervened by acupoint massage to press acupoints Hegu and Yemen. Then pain scores, blood pressure, heart rate, puncture success rate and satisfaction rate were observed and compared. Results After PICC insertion, systolic pressure and heart rate raised slightly in both groups, but were lower in patients of research group than those of control group(P<0.05). There was no difference in diastolic pressure of patients between two groups(P>0.05). Pain scores at different time points and puncture success rate had no difference between two groups(P>0.05). The satisfaction rate of research group was significantly higher than that of control group(100.00% vs. 94.00%, Z=-1.966, P=0.049). Conclusion Acupoint massage can effectively stabilize patient’s blood pressure and heart rate, relieve pain caused by PICC insertion and improve patient satisfaction.
