综合性护理干预对腺样体切除患儿术后疼痛及生活质量的影响

Effect of comprehensive nursing interventions on postoperative pain and quality of life in children undergoing adenoidectomy

目的探讨综合性护理干预对腺样体肥大切除术患儿术后疼痛及生活质量的影响。方法选取2018年1月—4月收治的88例行腺样体切除术治疗的患儿为研究对象,采用双盲抽签法分为观察组和对照组,各44例。对照组围手术期采用常规护理,观察组在对照组的基础上采用综合性护理。观察2组的术后1、3、5 d的疼痛评分,术后3 d儿童阻塞性睡眠呼吸暂停疾病特异性生活质量调查表(OSA-18)评分,评价患儿家属对护理的满意度。结果观察组术后1、3、5 d的Wong-bank面部表情量表法(FPS)评分均低于对照组,差异有统计学意义(P<0.05)。术后3 d观察组OSA-18量表睡眠障碍、情绪不佳、身体症状、白天功能、对监护人影响各维度得分均低于对照组,差异有统计学意义(P<0.05)。观察组患儿家属对护理安全、护理人员技术、手术信息、陪伴以及术后护理知识的满意度评分均高于对照组,差异有统计学意义(P<0.05)。结论综合性护理能够有效减轻腺样体肥大切除术患儿的术后疼痛,提高患儿术后生活质量和患儿家属的护理满意度。

Objective To investigate the effect of comprehensive nursing interventions on postoperative pain and quality of life in children undergoing adenoidectomy. Methods Totally 88 children undergoing adenoidectomy for adenoid hypertrophy were randomly divided into the observation group and the control group, with 44 cases in each group. Patients in both two groups received routing care during perioperative period, and those in the observation group were given comprehensive nursing interventions. The postoperative pain was evaluated by Wong-bank Faces Pain Scale(FPS). The quality of life was evaluated by Disease-specific Quality of Life for Children with Obstructive Sleep Apnea 18 Items Survey(OSA-18). The satisfactory degree of parents was evaluated by self-made questionnaire. Results The score of FPS in the observation group was lower than that in the control group at 1,3,5 days after surgery respectively(P<0.05). The scores of sleep disturbance, emotional distress, physical symptoms, daytime function, caregiver concerns of OSA-18 in the observation group were lower than those in the control group at 3 days after surgery(P<0.05). Parents of children in the observation group were more satisfied with the nursing safety, nursing techniques, surgical information, accompanying service and postoperative health education compared with those of children in the control group(P<0.05). Conclusion Comprehensive nursing interventions are potentially effective to reduce the postoperative pain and improve the quality of life in children undergoing adenoidectomy.
