摘要
目的:探讨无创通气治疗用于慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭患者的疗效。方法:选取2017年10月-2018年10月本院接收的COPD合并Ⅱ型呼吸衰竭患者60例。按照随机数字表法将其分为对照组(n=30,采取常规治疗)与观察组(n=30,采取常规治疗+无创通气治疗)。观察两组血气指标、肺功能指标(FVC、FEV_1及FEV_1/FVC)、呼吸频率(RR)、心率(HR)、呼吸困难严重程度的变化,比较两组住院时间。结果:治疗1、48 h后,观察组各项血气指标均优于对照组(P<0.05);治疗48 h后,观察组HR、RR均低于对照组(P<0.05);治疗后4周,观察组FVC、FEV_1及FEV_1/FVC均高于对照组(P<0.05);观察组治疗后4周呼吸困难症状评分均低于对照组(P<0.05);观察组住院时间短于对照组(P<0.05)。结论:对COPD合并Ⅱ型呼吸衰竭患者采取无创通气治疗,能显著改善患者的血气指标及肺功能,改善呼吸困难症状,缩短病程,值得于临床推广。
Objective:To discuss the efficacy of non-invasive ventilation in chronic obstructive pulmonary disease(COPD)with type Ⅱ of respiratory failure.Method:A total of 60 COPD patients with type Ⅱ of respiratory failure treated in our hospital from October 2017 to October 2018 were selected.According to the random number table method,they were divided into control group(n=30,conventional treatment)and observation group(n=30,conventional treatment+non-invasive ventilation treatment).The changes of blood gas index,lung function index(FVC,FEV1 and FEV1/FVC),respiratory rate(RR),heart rate(HR)and the severity of dyspnea in two groups were observed,and the hospitalization time of two groups was compared.Result:After 1 and 48 hours of treatment,the blood gas indexes of observation group were better than those of control group(P<0.05).After48 hours of treatment,the HR and RR in observation group were lower than those of control group(P<0.05).4 weeks after treatment,the FVC,FEV1 and FEV1/FVC in observation group were higher than those of control group(P<0.05).The scores of dyspnea symptoms in observation group were lower than those of control group at4 weeks after treatment(P<0.05).The hospitalization time of observation group was shorter than that of control group(P<0.05).Conclusion:The non-invasive ventilation for COPD patients with type Ⅱ respiratory failure can significantly improve the blood gas index and lung function,improve the symptoms of dyspnea and shorten the course of disease,which is worthy of clinical promotion.
