三绳固定法在口腔癌切除术加游离皮瓣修复术患者经口气管插管中的应用

Application of Three Wire Fixation in Oral Tracheal Intubation After Oral Cancer Resection Plus Free Flap Repairment

目的:探讨口腔癌切除+游离皮瓣修复术患者经口气管插管中三绳固定法的应用效果。方法:选取2015年5月-2017年10月在本院接受口腔癌切除术+游离皮瓣修复术治疗的口腔癌患者68例。按照不同经口气管插管固定方法将其分为对照组(n=32)和试验组(n=36)。试验组采用三绳固定法,对照组采用单绳固定法,比较两组气管插管移位情况、舒适度评分、并发症发生情况。结果:插管72 h后,试验组气管插管重度移位发生率为2.78%,低于对照组的18.75%(P<0.05);试验组固定后48、72 h的舒适度评分均高于对照组(P<0.05);试验组并发症发生率为8.33%,低于对照组的28.13%(P<0.05)。结论:对于行口腔癌切除术+游离皮瓣修复术治疗的患者,经气管插管施予三绳固定法可提升插管稳定性、患者舒适度,并减少并发症。

Objective:To investigate the effect of three wire fixation in oral tracheal intubation by oral cancer resection+free flap repairment.Method:A total of 68 patients with oral cancer who underwent oral cancer resection+free flap repair in our hospital from May 2015 to October 2017 were selected.According to different methods of oral tracheal intubation and fixation,they were divided into control group(n=32)and experimental group(n=36).The experimental group was fixed with three wire,and control group was fixed with single rope.The displacement of tracheal intubation,comfort score and complications between two groups were compared.Result:72 hours after intubation,the incidence of severe displacement of tracheal intubation in experimental group was 2.78%,which was lower than 18.75% of control group(P<0.05).The comfort scores of 48 and 72 hours after fixation in experimental group were higher than those of control group(P<0.05).The incidence of complications in experimental group was 8.33%,which was lower than 28.13% in control group(P<0.05).Conclusion:For patients undergoing oral cancer surgery+free flap repairment,three wire fixation through tracheal intubation can improve the stability of intubation,patient comfort and reduce complications.
