摘要
目的:分析无创呼吸机治疗高龄重症肺炎患者临床效果。方法:选取2016年12月-2017年12月到本院治疗的重症肺炎高龄患者72例为研究对象。按照随机数字表法将其分为试验组和对照组,各36例。对照组采取常规对症治疗方案联合有创呼吸机治疗,试验组采取常规对症疗治疗方案联合无创呼吸机治疗。比较两组肺功能情况(HR、PEEPi、PEF)、通气时长、住院时长、APACHEⅡ评分、呼吸频率(RR)、无创氧饱和度(SaO_2)以及血气分析指标(PaO_2、pH、PaCO_2)。结果:治疗后,试验组PEF高于对照组,HR、PEEPi均低于对照组(P<0.05);试验组通气时长、住院时长均短于对照组(P<0.05),试验组治疗后APACHEⅡ评分低于对照组(P<0.05);经机械通气治疗6 h后,试验组RR、PaCO_2均低于对照组,SaO_2、PaO_2、pH均高于对照组,差异均有统计学意义(P<0.05)。结论:无创呼吸机治疗高龄重症肺炎患者的临床效果确切,可有效改善患者心率、呼吸频率以及血气指标,缩短患者住院时间,同时操作简单方便,且创伤小,康复快。
Objective:To analyze the clinical effect of noninvasive ventilator in treatment of elderly severe pneumonia patients.Method:A total of 72 elderly patients with severe pneumonia who came to our hospital from December 2016 to December 2017 were selected as the study subjects.According to the random number table method,they were divided into experimental group and control group,36 cases in each group.The control group was treated with conventional symptomatic treatment combined with invasive ventilator,while experimental group was treated with conventional symptomatic treatment combined with noninvasive ventilator.The pulmonary function(HR,PEEPi,PEF),ventilation length,hospitalization length,APACHE Ⅱ score,respiratory rate(RR),noninvasive oxygen saturation(SaO2)and blood gas analysis index(PaO2,pH,PaCO2)between two groups were compared.Result:After treatment,the PEF in experimental group was higher than that of control group,HR and PEEPi were lower than those of control group(P<0.05).The ventilation length and hospitalization length in experimental group were shorter than those of control group(P<0.05),and the APACHE Ⅱ score in experimental group was lower than that of control group after treatment(P<0.05).After 6 hours of mechanical ventilation,RR and PaCO2 in experimental group were lower than those of control group,while SaO2,PaO2 and pH were higher than those of control group,the differences were statistically significant(P<0.05).Conclusion:The clinical effect of noninvasive ventilator in treatment of elderly patients with severe pneumonia is exact,it can effectively improve the heart rate,respiratory rate and blood gas index,shorten the hospitalization time of patients,at the same time,it is simple and convenient to operate,with less trauma and quick recovery.
