腰硬联合麻醉对妊娠期高血压疾病剖宫产产妇血液高凝及RAAS的影响观察

Influence of Combined Spinal-epidural Anesthesia for Blood Hypercoagulability and RAAS of Delivery Women with Cesarean Section for Hypertensive Disorder Complicating Pregnancy

目的:观察与研究腰硬联合麻醉对妊娠期高血压疾病剖宫产产妇血液高凝及RAAS的影响。方法:选取2017年1月-2019年1月本院收治的60例妊娠期高血压疾病剖宫产产妇作为研究对象,依据麻醉方式的不同分为对照组(连续硬膜外麻醉)30例和观察组(腰硬联合麻醉)30例。比较两组产妇围术期凝血指标、RAAS相关指标及血流动力学指标。结果:麻醉前两组凝血指标、RAAS相关指标及血流动力学指标比较,差异均无统计学意义(P>0.05),观察组术后即刻、术后6 h凝血指标及RAAS相关指标均低于对照组,术后即刻血流动力学指标优于对照组,差异均有统计学意义(P<0.05)。结论:腰硬联合麻醉对妊娠期高血压疾病剖宫产产妇血液高凝及RAAS的影响较好,在本类产妇中的应用价值相对更高。

Objective:To observe and study the influence of combined spinal-epidural anesthesia for the blood hypercoagulability and RAAS of delivery women with cesarean section for hypertensive disorder complicating pregnancy.Method:A total of 60 delivery women with cesarean section for hypertensive disorder complicating pregnancy from January 2017 to January 2019 were chosen as the study objects,and they were divided into the control group(continuous epidural anesthesia)30 cases and the observation group(combined spinal-epidural anesthesia)30 cases according to the difference of anesthesia types.Then the coagulation indexes,RAAS related indexes and hemodynamic indexes of two groups during the perioperative period were compared.Result:The coagulation indexes,RAAS related indexes and hemodynamic indexes of two groups before the anesthesia were compared,there were no statistically significant differences(P>0.05),the coagulation indexes and RAAS related indexes of the observation group at once after the operation and 6 h after the operation were lower than those of the control group,the hemodynamic indexes at once after the operation were better than those of the control group,there were statistically significant differences(P<0.05).Conclusion:The influence of combined spinal-epidural anesthesia for the blood hypercoagulability and RAAS of delivery women with cesarean section for hypertensive disorder complicating pregnancy are better,and its application value in these delivery women is relatively higher.
