皮肤修复膜在甲状腺、颌下腺手术中的应用观察(附16例报告)

The application observetion of the skin repair membrane in thyroid and salivary gland surgery.(enclose 16 cases report)

目的探讨在行甲状腺、颌下腺手术时创面不能用自身包膜修复时使用皮肤修复膜修复的效果观察,并对暴露的喉返神经使用皮肤修复膜进行保护后的效果观察。方法行患侧甲状腺全切加峡部切除术后,暴露喉返神经的5例,应用皮肤修复膜进行贴附保护。行甲状腺部分切除者,因为甲状腺切除后形成的创面使用本身包膜修复困难的6例,乃使用皮肤修复膜贴附创面,周围间断缝合修复。颌下腺部分切除的患者,因为肿物切除后本身包膜不能修复创面的5例,乃使用皮肤修复膜贴附创面,周围间断严密缝合。结果进行甲状腺手术保护创面和喉返神经保护的患者,顺利愈合,无喉返神经麻痹发生;颌下腺手术的愈合良好,无唾液漏及舌下神经麻痹发生。经过3个月-1年的随访,没有皮肤切口感染和过敏及其他并发症发生。结论在甲状腺、颌下腺手术时,如果创面不能用本身的包膜修复,可以使用皮肤修复膜修复。喉返神经暴露后可以使用皮肤修复膜保护,效果良好。

Objective To explore the efficacy observation of the skin repair membrane in thyroid and salivary gland surgery when surgical wound can not rpaired with their envelope.And explore the efficacy observation of that protective effection of the skin repair membrane to exposed recurrent laryngeal nerve.Methods There are 5 cases used the skin repair membrane to protection with recurrent laryngeal nerve were exposed in the surgery that affected side and isthmus of thyroid were excisioned.There are 6 cases that partial thyroidecyomy and their surgical wound rpaired very difficult with their envelope,used the skin repair membrane to attaching the wound and interrupted suture aroud.5 cases of partial salivary gland were excision can not rpaired with their envelope after the goitre of salivary gland excisioned.Results The patient who proceed thyroid surgery use the skin repair membrane to protection the wound and recurrent laryngeal nerve,and there was no recurrent laryngeal nerve paralysis.The salivary gland surgery healed well,there was no salivary fistula and hypoglossal nerve salivary.These cases were followed up for 3 month to one year,there was no skin infection,allergy and other complications.Conclusions If the surgical wound can not rpaired with their envelope and recurrent laryngeal nerve were exposed in thyroid and salivary gland surgery,we can use the skin repair membrane to repair and protection,the result is very well.
