Experience of professor YU Ying’ao in the treatment of pediatric epilepsy

余瀛鳌教授认为小儿癫痫主要病机为痰与瘀,病位在心、肝、脾、肾。小儿脾肾不足、心肝有余,若先天肾精不足,则脑髓失养;脾失健运,则痰湿内阻;心肝火旺,煎熬津液为痰,则夹痰上扰神明,致脑络瘀阻,神机失守而发病。治疗以潜镇止痫、化痰通络为法,临床给予癫痫促效方,疗效显著。同时余教授指出,癫痫在急性期,由于发作频繁,宜用汤剂控制;待病情稳定后,可予丸剂或散剂,以巩固疗效。方中金石之药较多,需固护小儿的脾胃之气,严格控制药物剂量,嘱服药6剂后停1日。附案例2则,以资验证。

Professor YU Ying’ao holds that the main pathogenesis of pediatric epilepsy lies in phlegm and stasis,and the location of disease is heart,liver,spleen and kidney.The children’spleen and kidney are deficient,while their heart and liver are redundant.If they are congenitally insufficiency of kidney essence,then the brain and the spinal cord are of nourishment loss;the spleen failing in transportation results in stagnation of phlegm-dampness;heart-liver with effulgent fire burns the fluid and humor to phlegm,and carries the phlegm upper and disturbs the bright spirit,which results in brain collateral stasis and obstruction and the onset due to the fall of state of mind.The therapeutic method is tranquilizing and hyperactivity-subduing to arrest epilepsy as well as resolving phlegm to dredge collaterals.Clinically he gives Dianxian Cuxiao Fang(癫痫促效方),which has obtained significant curative effects.At the same time,professor YU points out that epilepsy in the acute phase should be controlled by decoction due to frequent seizures;when the condition is stable,pills or powder can be given to consolidate the curative effect.There are many metal and mineral drugs in prescription,so it is necessary to reinforce the qi of spleen and stomach in children.Strictly controlling the dosage of drugs,six doses of drugs were ordered and stopped for one day.Two cases are attached for verification.
