中西医结合诊疗126例儿童大叶性肺炎总结体会

Diagnosis and treatment summary of combined traditional Chinese and Western medicine on 126 cases of lobar pneumonia in children

目的总结儿童大叶性肺炎中西医结合诊治特点,以提高对儿童大叶性肺炎的临床认识和诊治水平。方法对宜兴市中医医院儿科2017年1—5月收治的126例大叶性肺炎患儿的临床资料进行回顾性分析。结果 126例大叶性肺炎患儿以3~6岁为主,平均(5.3±3.2)岁;平均住院时间(8.67±3.19) d,住院超过10 d的有30例,总计住院费用640098.6元,平均住院费用(5080.15±1850.35)元;病原学检测中,肺炎支原体抗体IgM检测在呼吸道病原体九联检阳性率为58.11%,呼吸道病原体五联检阳性率为21.95%,外送阳性率为33.33%,总阳性率为44.63%;临床痊愈73例,显效27例,有效22例,无效4例,总有效率为96.83%。结论多药联用,中西医结合,内外合治,不仅有助于大叶性肺炎患儿尽快缓解病情,而且可以减少抗菌药物的使用及耐药菌的产生,同时治疗过程中积极针对并发症有效调整治疗方案,可取得较为理想的疗效。

Objective To summarize the characteristics of diagnosis and treatment of combined traditional Chinese and Western medicine on lobar pneumonia in children,and to improve the clinical understanding and diagnosis level of lobar pneumonia in children.Methods The clinic data of 126 cases of lobar pneumonia in children admitted to pediatrics department of Yixing Municipal Hospital of TCM from January to May in 2017 were retrospectively analyzed.Results Totally 126 children with lobar pneumonia were mainly 3-6 years old,with the average age(5.3±3.2)years and average length of hospital stay(8.67±3.19)days,30 cases hospitalized for more than 10 days.The total hospitalization cost was 640098.6 yuan,and the average hospitalization cost was(5080.15±1850.35)yuan.In the pathogenic detection,the positive rate of mycoplasma pneumoniae antibody IgM in joint inspection of nine respiratory pathogens was 58.11%,that in joint inspection of five respiratory pathogens was 21.95%,positive rate of delivery was 33.33%,and the total positive rate was 44.63%.Seventy-three cases were cured clinically,27 cases were markedly effective,22 cases were effective and 4 cases were ineffective and the total effective rate was 96.83%.Conclusion Multi-drug combination,combination of traditional Chinese and Western medicine,and internal and external treatment,which not only helps to alleviate lober pneum Onia in children as soon as possible,but also reduces the use of antibiotics and the generation of drug-resistant bacteria,at the same time,it actively adjusts the treatment program for complications effectively in the course of treatment,and can achieve a better effect.
