固精益肾方联合西药治疗肾病综合征的近期疗效及对血清免疫球蛋白及补体的影响

Effect of Gushen Yijing Formula Combined with Western Medicine on Nephrotic Syndrome and Its Effects on Serum Immunoglobulin and Complement

目的:观察固精益肾方联合西药治疗肾病综合征(Nephrotic Syndrome,NS)的近期疗效及对机体免疫球蛋白(Ig)及补体的影响。方法:选取NS患者86例,随机分为两组。对照组给予常规西药治疗,观察组给予固精益肾方联合常规西药治疗。比较两组患者治疗前后肾功能指标(24 hPro、ALB、Scr、BUN、Ccr)、免疫指标(IgA、IgM、IgG及补体C3、C4)、各时段中医证候积分及临床疗效。结果:观察组的临床总有效率明显高于对照组,两组比较有统计学意义(P<0.05)。治疗后观察组24 hPro、BUN、Scr明显降低(P<0.05),Ccr、ALB明显升高(P<0.05),并且各项指标较对照组的变化幅度大,两组比较具有统计学意义(P<0.05);治疗后观察组IgM、IgG均明显升高(P<0.05),IgA、C3明显降低(P<0.05),观察组较对照组变化幅度大(P<0.05);而两组C4水平均没有明显变化(P>0.05)。两组治疗1、2、3个月后中医证候积分均显著降低(P<0.05),积分减少率明显增加,并且观察组各时段较对照组证候积分降低更为显著(P<0.05)。观察组治疗过程中出现的不良反应明显少于对照组(P<0.05)。结论:固精益肾方联合西药治疗NS可以有效降低蛋白尿,改善患者肾功能,提高患者免疫功能,安全性高,值得推广使用。

Objective:To observe the effect of Gushen Yijing Formula combined with western medicine in the treatment of nephrotic syndrome(NS)and its effect on the body’s immunoglobulin(Ig)and complement.Methods:Eighty-six patients with NS were randomly divided into two groups.The control group was given conventional western medicinewhile the observation group was given Gushen Yijing Formula combined with routine western medicine.The renal function indexes(24 hPro,ALB,Scr,BUN,Ccr),immune indexes(IgA,IgM,IgG and complement C3,C4)before and after treatment were compared between the two groups,and the TCM syndrome scores and clinical efficacy at each time period were compared.Results:The total effective rate of the observation group was significantly higher than that of the control group,and the two groups had statistical significance(P<0.05).After treatment,24 hPro,BUN and Scr were significantly decreased(P<0.05).Ccr and ALB were significantly increased(P<0.05),and the indexes of each index were significantly higher than those of the control group.The two groups were statistically significant(P<0.05),and the IgM and IgG of the observation group were significantly higher(P<0.05)after treatment(P<0.05).IgAdecreased significantly(P<0.05).The changes in the observation group were larger than those in the control group(P<0.05),while there was no significant change in the C4 level in the two groups(P>0.05).The TCM syndrome scores of the two groups were significantly reduced(P<0.05)after 1,2 and 3 months,and the reduction rate of the integral was significantly increasedand the scores of the syndromes in the observation group were significantly lower than those in the control group(P<0.05).The adverse reactions in the observation group were significantly less than those in the control group(P<0.05).Conclusion:Gushen Yijing Formulacombined with western medicine in the treatment of NS can effectively reduce the proteinuria,improve the renal function and immune functionof the patientsand have high safety.It is worthy of popularizing.
