清胃败毒汤联合米诺环素治疗牙周脓肿的疗效观察

Clinical Efficacy of Qingwei Baidu Decoction Combined with Minocycline in Treatment of Periodontal Abscess

目的:观察清胃败毒汤联合米诺环素治疗牙周脓肿的近期临床疗效及不良反应。方法:选取93例牙周脓肿患者,随机分为两组,对照组患者采用盐酸米诺环素软膏治疗,观察组在对照组基础上加以清胃败毒汤治疗,以1周为1个疗程,治疗3个疗程。检测两组患者治疗前后牙周指标、中医证候积分、炎性因子,比较两组临床疗效,观察两组患者治疗过程中出现的不良反应,并进行统计分析。结果:观察组总有效率显著高于对照组,两组比较有统计学意义(P<0.05)。两组患者治疗1周、3周后,牙周指标(SBI、GI、PD、MD、自发痛VAS评分及牙周袋溢脓评分)、中医证候各项积分、炎性指标(TNF-α、IL-1β及IL-8)均显著较治疗前降低(P<0.05),并且治疗后观察组较对照组的各项评分降低更为显著(P<0.05);对照组不良反应发生率为19.6%,观察组不良反应发生率为2.1%,两组比较有统计学意义(P<0.05)。结论:清胃败毒汤联合米诺环素治疗牙周脓肿疗效显著,可有效降低炎症反应,安全性高,值得临床推广。

Objective:To observe the short-term clinical efficacy and adverse reactions of Qingwei Baidu Decoction combined with minocycline in the treatment of periodontal abscess.Method:Ninety-three cases of patients with periodontal abscess were randomly divided into two groups.The control group were treated by minocycline hydrochloride ointment treatment and the observation groupon the basis was given Qingwei Baidu Decoctionand 1 week for a course of treatment,totally 3 courses.We detected two groups’periodontal index,TCM syndrome integral,inflammatory factors and compared two groups’clinical curative effect.The two groups’adverse reactions were observed.Results:The total effective rate of the observation group was significantly higher than that of the control groupand the comparison between the two groups was statistically significant(P<0.05).Two groups’periodontal indexes(SBI,GI and PD,MD,spontaneous pain VAS score and periodontal pocket overflow pus score),TCM syndrome integral,inflammatory indicators(TNF alpha,beta IL-1 and IL-8)after 1 and 3 weekswere significantly reduced(P<0.05),and the observation group’s decreased more significantlythan those of the control group(P<0.05).The incidence of adverse reactions was 19.6%in the control group and 2.1%in the observation group.The comparison between the two groups was statistically significant(P<0.05).Conclusion:Qingwei Baidu Decoction combined with minocycline is effective in the treatment of periodontal abscess.
