潜阳安神汤对偏头痛伴焦虑、抑郁状态患者临床疗效及炎性细胞因子影响动态研究

Dynamic Study on Clinical Effect and Inflammatory Cytokines of Qianyang Anshen Decoction in Migraine Patients with Anxiety

目的:观察潜阳安神汤对偏头痛伴焦虑、抑郁状态患者临床疗效及炎性细胞因子影响。方法:80例偏头痛伴焦虑、抑郁患者随机分为对照组(口服盐酸氟桂利嗪胶囊,40例),观察组(对照组基础上口服潜阳安神汤,40例),观察两组患者治疗前及疗程结束后偏头痛综合评分、血清炎症因子TNF-α(Tumor Necrosis Factor)、IL-6(Interleukin-6)、CGRP(calcitonin gene related peptide)及血清5-羟色胺(5-hydroxytryptamine,5-HT)、焦虑(self-rating anxiety scale,SAS)、抑郁(self-rating depression scale,SDS)评分差异,并于治疗结束后1月、3月对所有患者进行追踪随访。结果:①与治疗前比较两组患者治疗结束后偏头痛综合评分明显下降,且观察组明显优于对照组(P<0.05),差异具有统计学意义。②疗程结束后两组患者TNF-α、IL-6、CGRP水平较治疗前明显下降,5-HT增加,且观察组优于对照组(P<0.05),差异具有统计学意义。③两组患者治疗结束后SAS、SDS评分与本组治疗前比明显下降,且观察组优于对照组(P<0.05),差异具有统计学意义。④治疗结束后1月、3月动态随访两组患者偏头痛综合评分、TNF-α、IL-6、CGRP、5-HT、SAS、SDS指标改善程度观察组仍优于对照组(P<0.05),差异具有统计学意义。结论:潜阳安神汤能够有效改善偏头痛患者临床症状,机体炎症反应,焦虑抑郁状态,促进患者康复。

Objiective:To observe the effect of Qianyang Anshen Decoction on clinical efficacy and inflammatory cytokines in migraine patients with anxiety.Methods:Eighty patients with migraine and anxiety were randomly divided into control group(oral flunarizine hydrochloride capsule,40 cases)and observation group(oral Qianyang Anshen Decoction,40 cases)to observe the comprehensive score of migraine,serum inflammatory factor(TNF-α),Interleukin-6(IL-6),calcitonin gene related peptide(CGRP),5-hydroxytryptamine(5-HT)and self-rating anxiety scale(SAS),self-rating depression scale(SDS)scores before and after treatment.All patients were followed up for 1 and 3 months after treatment.Results:①Compared with that before treatment,the comprehensive score of migraine of the two groups decreased significantly after treatment.The observation group’s was significantly better than that of the control group(P<0.05).②After treatment,the levels of TNF-α,IL-6 and CGRP in the two groups decreased significantly,and 5-HT increased especially in the observation group(P<0.05).③After treatment,SAS and SDS scores of the two groups were significantly lower than those before treatment,and the observation group’s was better than that of the control group(P<0.05).④The comprehensive scores of migraine,TNF-α,IL-6,CGRP,5-HT,SAS and SDS in the observation group were still better than those in the control group(P<0.05).Conclusion:Qianyang Anshen Decoction can effectively improve the clinical symptoms,inflammation,anxiety and depression of migraine patients and promote their rehabilitation.
