从血瘀“血变”辨析血液病血小板减少并发血栓栓塞

Differentiation of Thromboembolism And Thrombocytopenia In Hematological Diseases From Blood Stasis "Blood Change"

目的:从中医血瘀"血变"角度及现代医学血栓理论,对血液病血小板减少并发血栓栓塞进行辨析。方法:回顾10年来江苏省中医院血液科住院患者中血小板减少并发血栓的资料,对其生理特征及血栓高危因素加以分析,并以中医血瘀理论从病名、病因病机、病证特点、治则治法方面进行辨析。结果:高龄、深静脉置管、输血、糖皮质激素、化疗、血液病本病均为血栓栓塞的高危因素。结论:血液病患者在血小板减少时极易并发血栓栓塞,在治疗过程需时刻警惕血栓的风险,中医血瘀与现代医学血栓理论相结合,为血小板减少伴血栓栓塞的诊治提供了理论及实践依据。

Objective:From the perspective of blood stasis'blood change'in TCM and the theory of thrombosis to study the thromboembolism of thrombocytopenia in hematologic patients.Methods:Review the data of in-patients hematologic patients with thromboembolism of thrombocytopenia for 10 years,use the TCM theory of blood stasis and modern medical theory of thrombosis to analyze the clinical biological characteristics and the data of related risk factors of complicating thrombosis from the etiology,pathogenesis,the characteristics of the disease and treatment.Results:Advanced age,deep vein catheter,blood transfusion,glucocorticoids,chemotherapy and hematologic were high risk factors for thrombosis.Conclusion:Patients with thrombocytopenia in hematologic are prone to thromboembolism,so the risk of thrombus should be vigilant in the treatment process.The combination of TCM blood stasis and modern medical thrombus theory,will provide theoretical and practical basis for the diagnosis and treatment of thromboembolism.
