辨体-辨病-辨证诊疗模式在甲状腺功能减退症防治中的应用

Application of constitution differentiation-disease Differentiation-Syndrome differentiation diagnosis and treatment model in the prevention and treatment of hypothyroidism

甲状腺功能减退症是由于甲状腺激素合成和分泌减少或组织作用减弱导致的全身代谢减低综合征,严重危害人民健康。中医体质学说的发展为甲减的诊疗提供了新的方法。"辨体-辨病-辨证诊疗模式"是基于体病相关与体质可调理论的基础上,将辨体、辨病、辨证相结合应用于临床的中医诊疗方式。本文系统整理魏军平教授运用辨体-辨病-辨证诊疗模式诊断及治疗甲状腺功能减退症中的经验,主张临证时首先辨病确定其中西医病名,其次辨体预估疾病发展方向,然后辨证确立具体证型。治疗方面,注重在准确辨病的基础上,在甲减症状及体征明显的阶段以辨证论治为主兼以调体,症状基本缓解后转以调理偏颇体质为主,从而达到改善症状、调整体质进而防治甲减的目的。

Hypothyroidism is a systemic metabolic degeneration syndrome caused by decreased synthesis or secretion of thyroid hormones or weakened tissue effects,which seriously endangers people’s health.The development of TCM constitution theory has provided new ideas for the diagnosis and treatment of hypothyroidism.The model of"constitution differentiation-disease differentiation-syndrome differentiation"is a new Chinese medicine model based on the theory of physical disease-related and physical fitness,which combined constitution differentiation,disease differentiation and syndrome differentiation.This paper systematically organizes Professor WEI Jun-ping’s experience in the diagnosis and treatment of hypothyroidism using the model of"constitution differentiation-disease differentiation-syndrome differentiation".He advocates the disease differentiation first,the second constitution differentiation,and then the syndrome differentiation.In terms of treatment,Professor WEI Jun-ping advocates that on the basis of accurate disease differentiation,the syndrome differentiation is the main treatment,assisting to regulate physical fitness in the stage of hypothyroidism and signs.After the symptoms are basically relieved,it is mainly to adjust the biased body to achieve the purpose of improving symptoms,adjusting physical fitness and preventing hypothyroidism.
