Experience of Professor FANG Bangjiang in the Treatment of Gout

方邦江教授认为痛风应从标与本2方面来看待。标主要关乎于痰、毒、瘀、热,其病机主要与饮食起居失宜,风寒湿热外侵有关,本则责之肝、脾、肾。正虚卫外不固为发病的内在基础,感受外邪是发病的外在条件。对于痛风的辨证施治,可予清利湿热除痹、化瘀通络止痛、培补脾肾扶助正气等为治法,总以祛邪通络为治疗原则,应注意攻与补的先后与度,结合疾病各期的病机特点,临床中需详细诊查后辨证施治,并强调了预防与调摄的重要性。

Professor FANG Bangjiang believes that gout should be treated from both symptoms and roots aspects.Symptoms are mainly related to phlegm,poison,blood stasis,heat,and the pathogenesis is mainly related to inappropriate diet and living,wind,cold,dampness and heat invasion.While the roots are mainly about liver,spleen and kidney.Vital qi deficiency with defensive qi vacuity-exterior insecurity is the internal basis of the disease,and the external condition of the disease is to be infected by the exogenous pathogens.For the treatment based on syndrome differentiation of gout,it can be treated by clearing heat,eliminating dampness and removing arthralgia,dispelling the blood stasis and clearing collaterals to relieve pain,or supplementing spleen and kidney to cultivate vital qi,etc.The principle of treatment is to dispel pathogenic factors and clear collaterals.Attention should be paid to the sequence and extent of attacking and tonifying.Considering the pathogenesis of each stage of the disease,clinical diagnosis and treatment should be made carefully.In addition,the importance of prevention and regulation should be emphasized.
