老年人照护等级评估系统的标准照护时间与决策树模型及其政策应用研究 被引量:4

A Study of Standard Care Time and Decision Tree Model of the Rating System of Elderly Care and Its Policy Application

摘要 本研究基于评估研究的理论与方法,探索了我国长期照护保险制度照护等级评估系统的评估标准与评估工具及其实施举措。在评估研究科学性的效度原则下,收集机构老年人各类照护内容及其照护时间,测评老年人五个方面的身心综合状况,拟合出饮食照护时间、排泄照护时间、清洁照护时间的决策树模型,推算出一天24小时内直接生活照护的标准照护时间为20-51分钟,其中饮食11.00-15.33分钟、排泄2.67-11.33分钟、清洁6.33-26.67分钟。在评估研究政策性的目标原则下,将直接生活照护划分为三个等级,20-39分钟为轻度约占25%,40-49分钟为中度约占60%,50分钟以上为重度约占15%,并设计评估系统应用过程中保证公平性与效率性以及预防个人因素干扰的实施方案。 Using the theories and methods of assessment research,this paper makes a study of the evaluation criteria and tools and their implementation measures of the care grade evaluation system of our country's long term care insurance system.According to scientific validity principle of assessment research,after collecting various care contents and care time of the elderly in institutions,assessing five aspects of physical and mental comprehensive status of the elderly,and designing decision tree model of dietary care time,excretory care time and cleaning care time,it is calculated that within 24 hours a day,the standard care time for direct living care is 20-51 minutes,including 11.00-15.33 minutes of diet,2.67-11.33 minutes of excretion,and 6.33-26.67 minutes of cleaning.According to policy objective principle of assessment research,the direct living care is divided into three grades:mild care taking 20-39 minutes,accounting for about 25%of the investigated elders,moderate care taking 40-49 minutes,accounting for about 60%,and intense care taking more than 50 minutes,accounting for about 15%.The study also designs implementation plans to ensure fairness and efficiency and prevent personal interference in application of the evaluation system.
