临床药师参与1例冠状动脉介入术后合并消化道出血患者的用药监护

Pharmaceutical Care for One Case of Coronary Heart Disease with Gastrointestinal Bleeding by Clinical Pharmacists

临床药师参与1例经皮冠状动脉介入(PCI)术后合并消化道出血患者的治疗,协助临床医师制定个体化的抗血小板治疗方案。临床药师根据患者的消化道出血严重程度对临床医师的抗血小板方案提出建议,对患者用药疗效和不良反应进行药学监护,最后,患者病情好转,生命体征平稳。临床药师参与到临床抗血小板的治疗团队中可更好的保障临床合理用药。

Clinical pharmacists participated in the treatment of a patient with gastrointestinal bleeding after percutaneous coronary intervention(PCI),and assisted clinicians in formulating the individualized anti-platelet therapy.Clinical pharmacists provided reasonable recommendations according to the severity of the patient’s gastrointestinal bleeding and the subsequent anti-platelet plan,assisted doctors in monitoring the drug efficacy and adverse reactions,and gave the individualized medication guidance to the patient.Finally,under the supervision of clinical pharmacist,the patient’s vital signs were stable during hospitalization.The participation of clinical pharmacists in the clinical anti-platelet treatment team can better guarantee the rational clinical medication.
