翼状胬肉切除联合角膜缘干细胞移植术对眼表功能影响的临床观察

Clinical Observation the Function of the Ocular Surface of Pterygium Excision Combined with Limbal Stem cell Transplantation

目的探讨翼状胬肉切除联合角膜缘干细胞移植术对眼表功能的影响。方法选取无并发症的原发性翼状胬肉患者146例(146只眼),施行翼状胬肉切除联合角膜缘干细胞移植术,所有患者术后均随访1个月。分别于术前1天及术后1个月行术眼泪液分泌试验(Schirmer I test)及泪膜破裂时间(BUT)检查并比较。结果患者术眼Schirmer I试验术前为(7.51±1.62)mm/5min,BUT术前为(8.91±1.65)s,均低于正常。而在施行翼状胬肉切除联合角膜缘干细胞移植术后,患者术眼Schirmer I试验术后1个月为(11.71±2.88)mm/5min,BUT术后1个月为(10.59±1.71)s,均较术前明显延长,差异有统计学意义(P<0.01)。结论翼状胬肉可对眼表功能造成了明显损害,而翼状胬肉切除联合角膜缘干细胞移植术可提高泪膜的稳定性,改善眼表功能,是治疗翼状胬肉的一种安全有效的手术方式。

Objective To observation the effect of ocular surface function after pterygium excision combined with limbal stem cel transplantation.Methods 146 eyes of 146 pterygium patients without complications were undergone pterygium excision combined with limbal stem cel transplantation. Al patients were folowed-up for 1 months.Schirmer I test and BUT were recorded and compared between preoperative and postoperative 1 months.Results The Schirmer I test of preoperative was (7.51±1.62) mm/5min, the BUT of preoperative was (8.91±1.65) s, it was lower than normal. But after pterygium excision combined with limbal stem cel transplantation, The Schirmer I test of postoperative 1 months was(11.71±2.88)mm/5min,the BUT of postoperative 1 months was(10.59±1.71)s,it was significantly longer than preoperative(P<0.01).Conclusion Ocular surface function is damaged significantly by pterygium.Pterygium excision combined with limbal stem cel transplantation as one of the security and effective methods,it is abled to improve the stability of the tear film and ocular surface function.
