局部应用抗生素缓释系统治疗创伤后骨感染的疗效观察 被引量:4

Efficacy Observation of Local Application with Antibiotic Sustained-release System in Post-traumatic Bone Infection

摘要 目的探讨局部应用抗生素缓释系统治疗创伤后骨感染患者的疗效。方法选取辽中区人民医院骨科2015年10月至2017年12月收治的104例创伤后骨感染患者作为研究对象,利用随机数字表法将其分为两组,每组52例,对照组患者静脉滴注敏感抗菌药物;观察组患者使用局部应用抗生素缓释系统。比较两组患者骨折愈合时间、骨腔愈合时间、再次手术率、感染清除率和总有效率。结果观察组骨折愈合时间和骨腔愈合时间均显著短于对照组(均P<0.05);观察组和对照组再次手术率依次为7.69%和30.77%,观察组显著低于对照组(P<0.05);观察组和对照组Ⅰ期感染清除率依次为82.69%和53.85%,Ⅱ期感染清除率依次为90.38%和67.31%,观察组Ⅰ期和Ⅱ期感染清除率均显著高于对照组(均P<0.05);观察组和对照组治疗的总有效率依次为98.08%和90.38%,观察组显著高于对照组(P<0.05)。结论局部应用抗生素缓释系统治疗创伤后骨感染临床疗效显著,可有效缩短骨折愈合时间,提高感染清除率,降低再次手术概率。 Objective To explore the efficacy of local application with antibiotic sustained-release system in the treatment of post-traumatic bone infections. Methods A total of 104 patients with post-traumatic bone infection admitted from October 2015 to December 2017 in Liaozhong District People’s Hospital were selected and randomly assigned to 2 groups, 52 cases in each group.The control group took the intravenous injection of antibacterial drugs;the observation group took the local application with antibiotic sustained-release system. The healing time of bone fracture and bone space, reoperation rate, infection clearance ratio and total effective rate were compared. Results The healing time of bone fracture and bone space in the observation group was significantly shorter than that in the control group(P<0.05). The reoperation rate in the observation group(7.69%) was significantly lower than that in the control group(30.77%)(P<0.05). The infection clearance ratios at stage Ⅰ and stage Ⅱ in the observation group(82.69%, 90.38%) were significantly higher than those in the control group(53.85%, 67.31%)(P<0.05). The total effective rate in the observation group(98.08%) was significantly higher than that in the control group(90.38%)(P<0.05).Conclusion Local application of antibiotic sustained-release system in the treatment of post-traumatic bone infection has a significant clinical effect. It can effectively shorten the healing time of fracture, improve the clearance rate of infection and reduce the probability of reoperation.
