真武汤联合洋地黄类药物治疗阳虚水泛证型心力衰竭的临床疗效

Clinical Efficacy of Zhenwu Decoction Combined with Digitalis in the Treatment of Congestive Heart Failure with Syndrome of Water Overflowing due to Yang Deficiency

目的探讨真武汤联合洋地黄类药物治疗阳虚水泛证型心力衰竭患者的临床疗效。方法选取2017年1月至2019年5月宜春市中医院收治的阳虚水泛证型心力衰竭患者70例作为研究对象,按随机数字表法分为观察组与对照组,各35例。对照组采用去乙酰毛花苷治疗,观察组则在对照组基础上联合真武汤治疗。比较两组患者中医证候疗效与治疗前后心功能指标[N末端脑钠肽前体(NT-proBNP)、左室射血分数(LVEF)、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)]变化情况。结果治疗后,观察组中医证候治疗有效率94.29%,与对照组的91.43%比较,差异无统计学意义(P>0.05);治疗后,观察组LVEF(50.80±2.03)%高于对照组的(45.80±2.23)%,且NT-proBNP、LVESD、LVEDD水平观察组分别为(1 540.80±80.48)ng/ml、(33.48±7.18)mm、(52.05±7.66)mm,对照组分别为(1 623.80±80.63)ng/ml、(38.63±7.85)mm、(59.25±8.12)mm,差异有统计学意义(P<0.05)。结论真武汤联合去乙酰毛花苷治疗阳虚水泛证型心力衰竭临床效果显著,可有效改善患者中医证候,提升心脏功能。

Objective To explore the clinical efficacy of Zhenwu decoction combined with digitalis in treating heart failure of syndrome of water overflowing due to Yang deficiency.Methods From January 2017 to May 2019,a total of 70 patients with congestive heart failure with syndrome of water overflowing due to Yang deficiency were selected as the study subjects.They were divided into observation group and control group according to random number table method,with 35 cases in each group.The control group was treated with deacetyl glucoside,while the observation group was treated with Zhenwu decoction on the basis of the control group.The curative effect of TCM syndromes and the changes of cardiac function indexes(NT-pro BNP,left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD)before and after treatment were compared between the 2 groups.Results After treatment,the total effective rate of TCM syndrome treatment in the observation group(94.29%)was comparable to that of the control group(91.43%),and the difference was not statistically significant(P>0.05).The LVEF level in the observation group was[(50.80±2.03)%]high.In the control group[(45.80±2.23)%],and NT-proBNP,LVESD,LVEDD levels[(1540.80±80.48)ng/ml,(33.48±7.18)mm,(52.05±7.66)mm]were lower than the control group[(1623.80±80.63)ng/ml,(38.63±7.85)mm,(59.25±8.12)mm],the difference was statistically significant(P<0.05).Conclusion Zhenwu decoction combined with deacetyl glucoside has remarkable clinical effect in treating heart failure of syndrome of water overflowing due to Yang deficiency.It can effectively improve the TCM syndromes of patients and enhance cardiac function.
