多发型浅表脂肪瘤样痣1例

A Case of Nevus Lipomatosus Cutaneous Superficialis

患者男,26岁。右腰部丘疹、结节、斑块20多年。皮肤科情况:右腰部见多发、正常肤色的丘疹、结节和10. 0cm×5. 5cm融合性斑块,形态不规则,表面不平呈脑回状,质地柔软,活动可,境界清楚。采取局麻下分次手术切除的治疗方案,首次切除大部分皮损。皮损组织病理:轻度角化过度,局灶性角化不全,皮突延长,真皮胶原纤维增粗,真皮浅层见团块状成熟脂肪细胞。诊断:多发型浅表脂肪瘤样痣。

A 26-year-old man presented with papules,nodules and a gradually increasing mass on the right side of his waist for 20 years. There was no reported local abnormalities problems before the appearance of the present mass. Systemic examination was noncontributory. Multiple papules and nodules of different size,an oblong-shaped,skincolored,cerebriform plaque,measuring 10. 0 cm × 5. 5 cm were noticed on the right side of his waist. There was no tenderness,ulceration,or induration. A few discrete,soft,skin-colored papules were also scattered around the lesion. Neither regional lymphadenopathy nor other cutaneous abnormalities were present. The protocol of several sessions of surgical therapy under local anesthesia was adopted.Histopathological examination of the excised specimen showed mild hyperkeratosis,focal paprkeratosis,elongated rete ridges,thickened collagen fibers and clumpy mature fat cells in the superficial dermis. Based on the clinical manifestations and histopathological features,the diagnosis of Nevus Lipomatosus cutaneous superficialis was made.
