联合治疗膝关节疣状静脉畸形1例

Treatment of Verrucous Haemangioma of Knee Joint Combined Laser:A Case Report

患者女,19岁。左膝关节内侧暗褐色丘疹、结节10余年,伴疼痛加剧1年。皮肤科情况:左膝关节内侧暗褐色斑丘疹、结节,表面粗糙,基底暗红色,中央疣状增生,质硬,轻压痛,周围出现卫星结节病灶。皮损组织病理示:表皮角化过度,棘层增厚,真皮可见大量增生的薄壁血管,管腔高度扩张,内含红细胞。诊断:疣状血管瘤。治疗:连续Nd:YAG激光联合595nm脉冲染料激光治疗3次,每次间隔时间4～6周,术后遗留浅表瘢痕,膝关节活动不受限。随访3年未复发,患者对疗效满意。

A 19-year-old female patient presented with dark brown papules on the inside of the left knee joint,nodules for 10 years,accompanied by increasing pain for one year.Dermatology showed dark brown maculopapular rash on the left side of the knee joint,nodules,rough surface,dark red base,central verrucous hyperplasia,hard,mild tenderness,satellite nodular lesions around. Histopathology of skin lesions showed excessive keratinization of the epidermis,thickening of the spinous layer,and a large number of hyperplastic thin-walled blood vessels in the dermis. The lumen was highly dilated and contained red blood cells. Diagnosis: sickle hemangioma. Treatment:Continuous Nd: YAG laser combined with 595 nm pulsed dye laser treatment for three times,each interval of 4 to 6 weeks,superficial scar left after surgery,knee joint activity is not limited. There was no recurrence after 3 years of follow-up,and the patient was satisfied with the effect.
