摘要
目的探讨超声仿真内镜技术Fly-Thru(FT)在胆道疾病中应用的可行性。方法采集并重建100例胆道梗阻性疾病患者FT图像,由2名不同年资超声医师对FT图像质量进行评分,分析胆道疾病的FT图像特点,并对病变诊断信心进行分级。结果 100例FT图像均成功重建,成功率100%(100/100)。胆道良恶性病变具有特征性的FT表现。2名医师对FT图像质量评价一致性较好(Kappa=0.659)。常规超声结合FT可以提高受试者诊断信心,低年资医师和高年资采用二维超声和二维超声联合FT的诊断信心差异均有统计学意义(χ2=37.131、51.514,P均<0.01)。结论采用FT技术可获得质量稳定的胆道疾病图像,胆道良恶性病变具有特征性的FT表现;FT结合二维超声,有利于增强临床医师对胆道疾病诊断信心。
Objective To investigate the feasibility of Fly-Thru(FT)in the diagnosis of biliary tract diseases.MethodsTotally 100 patients with obstructed biliary tract diseases underwent FT examination.All FT images were reviewed by two radiologists with different experience.The quality of FT images was scale,the FT characteristics of biliary tract diseases were analyzed,and the diagnostic confidence of FT for the diagnosis of biliary tract diseases were evaluated.Results The technical success rate of FT image reconstruction was 100%(100/100).Benign and malignant lesions of the biliary tract had characteristic FT manifestations.The consistency in evaluating FT images between two observers was better(Kappa=0.659).Two-dimensional ultrasound combined with FT could improve the diagnostic confidence,the difference in diagnostic confidence between low-grade and seniors physicians using two-dimensional ultrasound and two-dimensional ultrasound combined with FT was statistically significant(χ~2=37.131,51.514,both P<0.01).Conclusion The FT is able to achieve reliable and stable image with specific characteristics between benign and malignant biliary tract diseases.FT combined with two-dimensional ultrasound is beneficial to improve the confidence of clinicians in the diagnosis of biliary tract diseases.
