摘要
目的探究聊城地区基层医院产前系统超声筛查胎儿畸形的价值。方法选取2013年4月至2018年4月在聊城地区基层医院行产前系统超声筛查胎儿畸形的孕妇160例为研究对象。均进行早孕期(11~13^(+6)w)、中孕期(14~27^(+6)w)、晚孕期(28~40^(+6)w)系统超声检查。观察不同孕期胎儿畸形检出情况及妊娠结局。结果 160例孕妇经分娩或引产后证实胎儿畸形15例(9.37%),产前系统超声筛查共检出14例畸形(93.33%),漏诊1例(6.67%);无误诊病例。早孕期检出4例(26.67%),为神经系统畸形(2例)、骨骼肢体畸形(1例)、胸腹壁畸形(1例)。中孕期检出7例(46.67%),为颜面部畸形(2例)、骨骼肢体畸形(2例)、神经系统畸形(1例)、心脏畸形(1例)、泌尿系统畸形(1例)。晚孕期检出3例(20.00%),为神经系统畸形(1例)、泌尿系统畸形(1例)、胸腹壁畸形(1例)。漏诊1例心脏畸形之室间隔缺损于产后检出。15例畸形胎儿妊娠结局中,5例(33.33%)活产,10例(66.67%)引产。结论基层医院加强产前系统超声筛查胎儿畸形有助于提高胎儿畸形检出率,降低婴儿出生缺陷率,促进优生优育。
Objective:To investigate the value of ultrasound screening for fetal malformation in prenatal system in primary hospitals in Liaocheng area.Methods:A total of 160 pregnant women with prenatal ultrasound screening for fetal malformation in the primary hospital in Liaocheng area from April 2013 to April 2018 were selected as subjects.Ultrasound examination was performed in early pregnancy(11~13+6 weeks),middle pregnancy(14~27+6 weeks),and late pregnancy(28~40+6 weeks).Observe the detection of fetal malformation and pregnancy outcome in different pregnancy.Results:Fifteen pregnant women were diagnosed with fetal malformation by postpartum or induced labor in 15 cases(9.37%).Fourteen malformations(93.33%)were detected by prenatal system ultrasound screening,and one case(6.67%)was missed.There were no misdiagnosed cases.Four cases(26.67%)were detected in early pregnancy,which were neurological malformations(2 cases),skeletal limb malformations(1 case),and chest and abdominal wall deformities(1 case).7 cases(46.67%)were detected in the middle pregnancy,facial deformity(2 cases),skeletal limb malformation(2 cases),nervous system malformation(1 case),cardiac malformation(1 case),urinary system malformation(1 case).Three cases(20.00%)were detected in late pregnancy,which were neurological malformations(1 case),urinary system malformation(1 case),and chest and abdominal wall deformity(1 case).A ventricular septal defect with a missed diagnosis of cardiac malformation was detected postpartum.Of the 15 abnormal fetal pregnancy outcomes,5(33.33%)live births and 10(66.67%)induced labor.Conclusion:Strengthening prenatal system ultrasound screening of fetal malformation in primary hospitals can improve the detection rate of fetal malformation,reduce the birth defect rate of infants,and promote prenatal and postnatal care.
