摘要
目的探讨不同程度窒息对足月新生儿髋关节发育异常的影响及与分型的相关性。方法选择2016年3月-2018年2月治疗的窒息新生儿78例作为对象,设为观察组。根据新生儿阿氏评分分为轻度窒息组与重度窒息组。选择同期正常分娩的新生儿69例作为对象,设为对照组。两组均采用GElogC9彩色多普勒超声诊断仪对两组髋关节发育情况进行测定;统计并记录两组测量角a、β;根据测定结果依据Graf分型标准对患者髋关节发育异常进行分型;采用SPSSPearson相关性分析软件对不同窒息程度与足月新生儿髋关节发育分型进行相关性分析。结果观察组髋关节中测量角α,低于对照组(P<0.05);观察组髋关节中测量角β,均高于对照组(P<0.05);观察组中轻度窒息组髋关节中测量角α,高于重度窒息组(P<0.05);观察组中轻度窒息组髋关节中测量角β,低于对照组(P<0.05);轻度窒息组与重度窒息组Ⅰ型,Ⅱa型,Ⅱb型,Ⅱc型,Ⅱd型率,均高于对照组(P<0.05);轻度窒息组Ⅰ型、Ⅱa型率均高于重度窒息组(P<0.05);轻度窒息组Ⅱc型,Ⅱd、Ⅲ型与Ⅳ型率,均低于重度窒息组(P<0.05);观察组与对照组Ⅰ型测量角α,均高于Graf其他分型;观察组与对照组Ⅰ型测量角β,均低于Graf其他分型;SPSSPearson相关性分析结果表明:足月新生儿窒息程度与Ⅰ型,Ⅱa型,Ⅱb型,Ⅱc型,Ⅱd型,Ⅲ型及Ⅳ型呈正相关性(P<0.05)。结论窒息会对足月新生儿髋关节发育产生影响,窒息程度越高,发育异常率越高;窒息程度还与髋关节分型存在相关性;应及时采取措施预防或减轻新生儿窒息率,避免对髋关节发育产生影响。
Objective:To investigate the effects of different degrees of asphyxia on hip dysplasia in full-term neonates and their correlation with typing.Methods:A total of 78 neonates with asphyxia treated from March 2016 to February 2018 were selected as the observation group.According to the newborn A′s score,it was divided into mild asphyxia group and severe asphyxia group.69 cases of neonates who had normal delivery during the same period were selected as the control group.The GE logC9 color Doppler ultrasound diagnostic apparatus was used to measure the development of hip joints in both groups.The measurement angles a andβwere recorded and recorded.According to the results of the measurement,the hip joint development abnormalities were classified according to the Graf classification criteria.The SPSSPearson correlation analysis software was used to analyze the correlation between different levels of asphyxia and full-term neonatal hip development.Results:The measured angleαin the hip joint of the observation group was lower than that of the control group(P<0.05).The measured angleβin the hip joint of the observation group was higher than that of the control group(P<0.05).The hip group of the mild asphyxia group in the observation group The mid-measurement angleαwas higher than that in the severe asphyxia group(P<0.05).In the observation group,the measured angleβin the hip joint of the mild asphyxia group was lower than that of the control group(P<0.05);the mild asphyxia group and the severe asphyxia groupⅠType,typeⅡa,typeⅡb,typeⅡc,and typeⅡd were higher than those in the control group(P<0.05).The rate of type I and type IIa in mild asphyxia group was higher than that in severe asphyxia group(P<0.05);mild The rates of typeⅡc,Ⅱd,ⅢandⅣin asphyxia group were lower than those in severe asphyxia group(P<0.05).The angleαof the typeⅠin the observation group and the control group were higher than those in Graf;the observation group and the control group.TypeⅠmeasurement angleβwas lower than other Graf classifications;SPSSPearson correlation analysis showed that the degree of neonatal asphyxia was positively correlated with typeⅠ,typeⅡa,typeⅡb,typeⅡc,type IId,typeⅢand typeⅣ.Sex(P<0.05).Conclusion:Asphyxia can affect the development of hip joint in full-term neonates.The higher the degree of asphyxia,the higher the rate of developmental abnormality.The degree of asphyxia is also related to the classification of hip joints.Measures should be taken to prevent or reduce the rate of neonatal asphyxia.Avoid affecting hip development.
