摘要
目的 :研究分析纤维支气管镜(纤支镜)吸痰疗法联合抗感染治疗对重症肺部感染患者呼吸指标及炎性应激情况的影响。方法:选取2015年2月-2016年3月在我院接受治疗的重症肺部感染患者84例,按随机数字表法分为观察组及对照组,各42例。对照组给予常规抗感染治疗,观察组则在对照组基础上加用纤支镜吸痰治疗。比较两组疗效、呼吸指标、炎性因子、临床症状改善时间及住院时间。结果 :观察组治疗总有效率为92.86%,显著高于对照组的76.19%(P<0.05);治疗后观察组用力呼气容积(FEV1)、最大通气量(MMV)、最大呼气中期流量(MMEF)、功能残气量(FRC)、肺总量(TLC)水平均显著高于对照组(P<0.05);治疗后观察组血清血小板激活因子(PAF)、巨噬细胞炎症蛋白-1α(MIP-1α)、可溶性髓系细胞触发受体-1(s TREM-1)、降钙素原(PCT)均显著低于对照组(P<0.05),脂联素(APN)水平显著高于对照组(P<0.05);观察组体温恢复时间、血常规恢复时间以及住院时间均显著短于对照组(P<0.05)。结论 :纤支镜吸痰疗法联合抗感染治疗重症肺部感染的临床疗效显著,可有效改善患者呼吸指标及炎性因子水平,有利于促进患者早日康复。
Objective:To study the effect of bronchoscopy sputum aspiration therapy combined with anti-infection therapy on respiratory index and stress situation of inflammatory in patients with severe pulmonary infection. Methods:84 cases of severe pulmonary infection in our hospital from February 2015 to March 2016 were randomly divided into the observation group and the control group, 42 cases in each group. The control group was given conventional anti-infective therapy, observation group was treated with bronchoscopy sputum aspiration therapy based on the control group. The curative effect, respiratory index, inflammatory factors, improvement time of clinical symptoms and length of hospital stay were compared between the two groups. Results:The total efficiency of the observation group was 92.86% which was significantly higher than 76.19% in the control group(P<0.05). After treatment, forced expiratory volume(FEV1), maximal ventilatory volume(MMV), maximal mid expiratory flow(MMEF), functional residual capacity(FRC), total lung capacity(TLC) in observation group were significantly higher than those in control group(P<0.05). Platelet activating factor(PAF), macrophage inflammatory protein-1 alpha(MIP-1 alpha), soluble triggering receptor-1(s TREM-1) and procalcitonin(PCT) in observation group were significantly lower than those in the control group(P<0.05). Adiponectin(APN) level in observation group was significantly higher than that in control group(P<0.05). The recovery time of body temperature, recovery time of blood routine examination and hospitalization time in observation group were significantly shorter than those in the control group(P<0.05). Conclusion:The clinical curative effect of bronchoscopy sputum suction therapy combined with anti-infection therapy in the treatment of severe pulmonary infection is remarkable. The method can effectively improve the patient's respiratory index and inflammatory factors, and it is conducive to promoting early recovery of patients.
