我院静脉药物配置中心不合理医嘱分析

Analysis of Irrational Physician Orders in the Pharmacy Intravenous Admixture Services Center of Our Hospital

目的:通过对静脉药物配置中心(PIVAS)的不合理医嘱进行分析研究,为临床用药的合理、安全、有效提供参考。方法 :对我院PIVAS药师2016年7-12月集中配置的医嘱中审核发现的不合理医嘱进行统计分析。结果:我院PIVAS药师共审核出不合理医嘱229组,占总医嘱的0.27%,其中药物配伍不合理19组,药物溶媒选择不合理65组,药物稀释浓度不合理33组,药物用法用量不合理57组,其他不合理用药情况55组,反馈给临床后基本得到了及时修改。结论 :药师应对不合理处方进行干预,并不断提高医嘱审核能力,促进合理用药。

Objective:To examine the irrational physician orders in the intravenous admixture services(PIVAS) center in order to provide insights into rational,safe and effective clinical use of drugs.Methods:Statistical analysis was performed on irrational orders identified in the PIVAS pharmacist order review between July to December 2016 in our hospital.Results:A total of 229 irrational physician orders were identified from the PIVAS pharmacist review,which accounted for 0.27% of total physician orders given during that period.Among them,19 were unreasonable drug admixtures,65 were irrational choice of medicinal solvent,33 were unreasonable drug dilution concentration,57 were irrational drug dosage and use,and 55 were other irrational uses of drugs.These irrational practices were generally corrected once feedbacks were returned to the hospital.Conclusions:Pharmacist should intervene in irrational physician order and enhance the ability of examining prescription,which can improve the level of rational drug use in our hospital.
