参松养心胶囊联合胺碘酮治疗慢性心力衰竭伴室性心律失常的临床研究

Clinical Study of the Combination of Shensongyangxin Capsule with Amiodarone in the Treatment of Chronic Heart Failure with Ventricular Arrhythmia

目的 :探讨参松养心胶囊联合胺碘酮治疗慢性心力衰竭伴室性心律失常的临床疗效。方法 :将150例慢性心力衰竭合并室性心律失常患者按随机数字表法分为对照组和研究组,各75例。对照组给予口服胺碘酮(200 mg,tid,连续服用1周,之后改为200 mg,bid,连续服用1周,再调整为100 mg,qd,连续服用2周)治疗;研究组在对照组基础上联合参松养心胶囊(0.4 g×4,tid)治疗。统计分析两组临床疗效,分别于治疗前后测量室性早搏的总数、QT离散度(QTd)及血浆中B型氨基端利钠肽原(NT-pro BNP)水平,观察不良反应发生率。结果 :研究组临床总有效率明显高于对照组(P<0.05);与治疗前比较,两组患者治疗后室早总数、QTd及NT-pro BNP水平明显降低(P<0.05),同时研究组指标的改善情况显著优于对照组(P<0.05)。两组不良反应发生率差异无统计学意义(P>0.05)。结论 :参松养心胶囊联合胺碘酮治疗慢性心力衰竭伴室性心律失常的临床疗效显著,能够改善临床相关指标,安全性较高,值得在临床推广应用。

Objective:To explore the clinical curative effect of shensongyangxin capsule combined with amiodarone on chronic heart failure with ventricular arrhythmia. Methods:150 cases of patients surffering from chronic heart failure and ventricular arrhythmia were randomly divided into control group and research group, with 75 cases in each group. Patients in control group were treated with oral amiodarone(200 mg, tid for 1 week, then 200 mg, bid for 1 week,and then adjusted to 100 mg, qd for 2 weeks), while patients in the research group were treated with shensongyangxin capsule(0.4 g×4,tid) in addition to the treatment in control group. The clinical curative effect in two groups was analyzed. The total number of ventricular premature beat, QT dispersion degree(QTd) and plasma b-type natriuretic peptide amino end in the original(NT-pro BNP) level were measured respectively before and after treatment. The incidence of adverse reactions of patients was observed. Results:The total clinical effective rate of patients in the research group was significantly higher than that in the control group(P<0.05). Compared with the results before treatment, the total number of premature ventricular, QTd and NT-pro BNP level of patients in two groups after treatment significantly decreased(P<0.05). The improvement of the patient's indexes in the research group was significantly better than that in the control group(P<0.05). The difference of adverse reaction rate between the two groups of patients showed no statistical significance(P>0.05). Conclusion:Shensongyangxin capsule combined with amiodarone has remarkable curative effect in the treatment of chronic heart failure with ventricular arrhythmia by improving the clinical related indicators, with high safety, which is worth popularizing and applying in clinic.
