自拟祛痹汤合四妙散对风湿热郁型类风湿性关节炎患者免疫指标的影响

Effect of Self-made Qubi Decoction Combined with Simiao Powder on Immune Indexes of Patients with Wind-damp-heat Syndrorne Rheumatoid Arthritis

目的 :探讨自拟祛痹汤合四妙散对风湿热郁型类风湿性关节炎患者免疫指标的影响。方法 :选取2015年12月-2016年12月我院收治的风湿热郁型类风湿性关节炎患者72例为研究对象,按随机数字表法分为对照组与观察组,各36例。对照组给予常规西医治疗(美洛昔康片7.5 mg,qd;甲氨蝶呤片7.5 mg,qwd),观察组在对照组基础上给予自拟祛痹汤合四妙散(1剂/d,加水煎煮取汁,分早晚2次服用)治疗,30 d为1个疗程,共治疗2个疗程,比较两组治疗效果,记录两组晨僵时间、关节疼痛评分、关节肿胀缓解情况等,同时测定两组治疗前后免疫功能指标,包括类风湿因子(RF)及红细胞沉降率(ESR)等,并评价其用药安全性。结果:观察组治疗后晨僵时间短于对照组(P<0.05),关节疼痛评分显著低于对照组(P<0.05),关节肿胀缓解率显著高于对照组(P<0.05);观察组治疗后RF、ESR水平均低于对照组(P<0.05);观察组治疗总有效率高于对照组(P<0.05);观察组用药不良反应发生率低于对照组(P<0.05)。结论 :在常规西医治疗基础上联合自拟祛痹汤合四妙散治疗风湿热郁型类风湿性关节炎效果显著,可改善患者免疫功能。

Objective:To investigate the effect of self-made qubi decoction combined with Simiao powder on immune Indexes of patients with wind-damp-heat syndrome rheumatoid arthritis. Methods:72 cases of patients with wind-damp-heat syndrome rheumatoid arthritis treated in our hospital from December 2015 to December 2016 were selected as subjects and divided into control group and observation group according to the random number table method, 36 cases in each group. The patients in the control group were given conventional Western medicine(meloxicam tablet 7.5 mg, qd; methotrexate tablet 7.5 mg, qwd), and the patients in the observation group were given self-made qubi decoction combined with simiao powder(1 dose/d, decoction in water and taking juice, twice in morning and evening) on the basis of the control group, 30 days for one treatment course, a total of two treatment courses. The treatment effect of the two groups was compared; the morning stiffness time, joint pain score and joint swelling remission were recorded and at the same time, the immune function indexes, including rheumatoid factor(RF) and erythrocyte sedimentation rate(ESR), before and after treatment were measured; their safety was evaluated. Results:The morning stiffness after treatment in the observation group was shorter than that in the control group(P<0.05), the joint pain score in the observation group was lower than that in the control group(P<0.05), and joint swelling remission rate was higher than that in the control group(P<0.05). The RF and ESR levels after treatment in the observation group were lower than those in the control group(P<0.05); the total effective rate of the observation group was higher than that of the control group(P<0.05); the incidence of adverse drug reactions in the observation group was lower than that in the control group(P<0.05). Conclusion:Based on conventional Western medicine treatment, self-made qubi decoction combined with simiao powder is effective in treating wind-damp-heat syndrome rheumatoid arthritis, and improves the immune function of the patients.
